Benton County Is Now Actively Recruiting for And Accepting Applications to Fill a Full-Time Social Worker Position Within the Mental Health Unit for Our Human Services Department. Within This Unit, Primary Duties Include Adult Mental Health Case Management and Civil Commitment Screenings.

The Intended 2023 Hiring Wage for This Position Is $29.14 Per Hour; with a career potential of $39.97 Per Hour.

The primary caseload for this social worker position will be in the area of Adult Mental Health Case Management. This is a full-time position with a normal department schedule of 8:00 am - 4:30 pm, Monday through Friday. This Case Management position works with clients (both voluntary and those under civil commitment) who meet the criteria as having a severe and persistent mental illness.
This position will participate in the on-call rotation two weeks during the year. Additional compensation is provided when on-call.
Position Summary
The Social Worker’s primary purpose is to ensure health and safety of individuals we serve and provide person centered planning with an effort to enhance individual lives and to promote happiness, independence, and autonomy. Social Workers must determine eligibility for the programs coordinated and monitored by the County, as well as assisting clients in making informed choices regarding services. Determining eligibility includes but is not limited to conducting the assessment and gathering financial and any other information required to make that determination.

Essential Duties and Responsibilities

A. Assess clients to determine individual needs and establish treatment/service goals.
B. Maintain caseload and provide ongoing case management including crisis management.
C. Effectively interview and engage clients to assess and evaluate needs.
C.1. Interview and counsel client.
C.2. Make field visits to clients’ homes.
C.3. Explain policies, regulations, and programs to clients and the public.
C.4. Provide immediate response in crisis situations to maintain safety of clients and the community.
D. Develop and complete assessments and documentation, including but not limited to, social histories, home assessments, and individual or family service plans to treat identified problems.
E. Prepare intensive long or short-term treatment plans.
F. Manage individual clients or families to explore, identify, resolve, prevent or control problems.
G. Complete required paperwork and documentation as related to client activity according to mandated timelines.
H. Prepare and maintain case records which meet federal, state, and local guidelines.
I. Attend court hearings as required.
J. Update client information (SSIS).
K. Generate case plans and assessments to outline needs, current functioning levels and expectations of clients.
L. Facilitate access to and refer clients to appropriate community services.
M. Interprets policies and regulations to clients.
N. Provides casework services for rehabilitation and care of clients.
O. Facilitate access and provide information to individuals upon request, and/or refer them to appropriate community services/resources.
P. Follow through on treatment/service plans.
Q. Monitor progress toward goals and objectives.
R. Perform intake as assigned.
S. Handle on-call responsibilities on a rotating basis with other social workers.
S.1. Available to respond to weekend, holiday, or crisis situations, on-call as scheduled.
T. Other duties as assigned.

A. Attends trainings as needed.
B. Maintains accurate client records on computer system, prepares reports, and submits to proper personnel and/or agencies.
C. Prepares and maintains progress notes, narrative summaries, reports, and other required case documentation in the file and in SSIS.
D. Collaborates and consults with multidisciplinary and multi-agency teams in order to achieve desirable outcomes for clients.
E. Attends continuing education programs and participates in in-service training.
F. Maintains knowledge of best practices in the profession.
G. Works closely with law enforcement and county attorney staff in development and follow- up to court cases.
H. Consult with other agency units/workers when services/needs overlap programs.
I. Maintain professional boundaries.
J. Function as a contributing and productive member of the unit and agency by attending and participating in meetings.
K. Provide community education and other associated duties as assigned.
L. Participate and comply with social services on-call systems.
M. Be available for consultations and other crisis interventions during non-office hours.
N. Maintain paperless system for record keeping.
O. Performs other duties as assigned or apparent.

SOCIAL WORKER ADULT AND CHILD MENTAL HEALTH - ADDENDUM

A. Performs duties for adult mental health and children mental health targeted case management.
B. Adult duties include case management for voluntary and civilly committed clients.
C. Coordinate with mental health, housing, medical, educational, and vocation providers.
D. Monitor client compliance with terms of court orders, provisional discharges, and report to the courts, DHS, and State Forensics Services to ensure safety of client and community members.
E. Assist with completing chemical dependency assessments, as needed.
F. Create strategies to improve and monitor attendance to avoid court action.
G. Make mental health referrals.
H. Coordinate mental health services for clients.
I. Obtain resource information and provide to clients.
J. Serve as members of the commitment screening team and are responsible for decisions as to whether or not a client is referred to the county attorney for civil commitment.
K. Make decisions about whether or not clients are revoked from provisional discharges and stays of commitment.
L. Provide crisis intervention work with suicidal clients.
M. Research to find placement options for clients.
Additional Minimum Qualifications
A. Rule 79 Certification within one year of hire.

SOCIAL WORKER CHILD PROTECTION / CHILD SERVICES - ADDENDUM
A. Provide professional social worker services involved in assessments, evaluations, and implementation of child protection service plans to assist clients and ensure the safety of children.
B. Communicate effectively with community partners which includes law enforcement, mental health, school, medical, and court professionals.
C. Ensure the immediate safety and well-being of children.
D. Review reports of alleged case of neglect or abuse.
E. Screen to see if meets maltreatment guidelines.
F. Make field visits to clients’ homes.
G. Manage safety planning and placement of children outside the home.
H. Manage the Parent Support Outreach Program (PSOP) to assist at risk clients with voluntary services by providing connections to community organizations.
H.1. Address concerns impacting families, such as poverty, substance abuse, domestic violence, behavioral health concerns, and past history of abuse or neglect.
H.2. Reduce childhood abuse and neglect by providing clients with the resources they need.
H.3. Contact clients for rule of 25/AMH or CHM referrals.
Additional Minimum Qualifications
A. Child Protection Certification within six months of hire.
SOCIAL WORKER INTAKE - ADDENDUM
A. Maintain safety, permanency, and well-being of children in Benton County.
B. Take child protection reports and write up.
C. Write up child protection intake reports from faxed in reports from law enforcement, hospitals, educators, and other mandated reporters.
D. Check MGA, Maxis and SSIS for history in regards to new intakes.
E. Take Rule 25 Assessment and Commitment requests.
F. Write up child and adult mental health intakes.
G. Communicate effectively with community partners which include law enforcement, mental health, school, medical and court professionals.
H. Screen to see if reports meet maltreatment guidelines.
I. Intake is responsible for screening out reports.
J. Provide community resources to residents.
SOCIAL WORKER DISABILITY AND AGING SERVICES - ADDENDUM
A. Waiver case management services to individuals who are eligible for Alternative Care, Elderly Waiver, Community Alternative Care, Community Access for Disability Inclusion waiver, Rule 185, Consumer Support Grant, Family Support Grant, Semi-Independent Living Services, Relocation Services, and Essential Community Support includes but is not limited to:
A.1. Evaluate and determine services needed to support health and safety for persons with disabilities or aging population.
A.2. Educate individual receiving services, family members and community providers of services available through waiver programs.
A.3. Develop person-centered care plan.
A.4. Coordinate referrals and services.
A.5. Assist with access and navigation of social, health, educational, vocational, housing, and other community supports and services.
A.6. Manage individual service plan budgets within state guideline and cost effectiveness.
A.7. Monitor quality and effectiveness of services provided through waiver funding for the safety and well-being of the person receiving the services.
B. Develop, revise, update internal agency policies and procedures for Waiver Case Management as assigned.
C. Complete Adult Protection Investigation as assigned.
D. Assess health, risk, and safety of individuals.
E. Provide immediate response in crisis situations to maintain safety of clients in the community.
F. Aid and assist in the establishment of guardianship/conservatorship to protect the vulnerable adult.
Additional Minimum Qualifications:
A. DHS Person Centered Support Plan Training within one year of hire.
B. Vulnerable Adult Training within one year of hire.
DD Case Management Only:
A. Minimum one year of experience in education or treatment of people with developmental or related conditions or a minimum of one course that specifically focuses on developmental disabilities.
B. Twenty (20) hours per year of training in the area of case management or developmental disabilities.
SOCIAL WORKER MN CHOICE ASSESSOR - ADDENDUM
A. Coordinate services for Consumer Support Grant recipients.
B. Determine individual needs and eligibility for county, state, and federal programs through MN Choices assessments.
C. Complete additional Rule 185 (Developmental Disabilities) case management eligibility as a part of ongoing MNCHOICES assessment.
D. Take on projects as assigned to develop, revise and update policies and procedures.
E. Attend MNCHOICES meetings with local counties.
F. Prepare for, respond to, and represent Benton County at court appeal hearings.
G. Complete all paperwork included with MNCHOICES Assessment.
H. Complete the residential services tool if the person is living in a customized living setting and will be accessing EW.
I. Determine individuals needs and eligibility for 11 different counties, state, and federal programs.
J. Provide professional, accurate, and timely assessments for clients to determine individual needs.
K. Maintain early intervention log.
Additional Minimum Qualifications
A. MNCHOICE Certification within six months.
SOCIAL WORKER ADULT PROTECTION - ADDENDUM
A. Protect and promote safety, independence, and quality of life order.
B. Assist in providing safe environments for adults.
C. Petition court to emergency and full guardianship and conservatorship.
Additional Minimum Qualifications
A. Adult Protection training within one year of hire, with eight hours on-going training per year.
B. MN CHOICE Certification within six months of hire.

A. Assess clients to determine if there is a substance use disorder and diagnose accordingly.
B. Make referral to either outpatient or inpatient programs.
C. Manage the caseload, create service plans, fill out client placement authorization for funding purposes.
D. Review documentation consulting and screening with Mental Health team.
E. Write petition and submit to attorney.
F. Transport clients to placement, services, appointments, etc.
G. Serve as members of the commitment screening team and are responsible for decisions as to whether or not a client is referred to the county attorney for civil commitment.
H. Make decisions about whether or not clients are revoked from provisional discharges and stays of commitment.
I. Provide crisis intervention work with suicidal clients.
J. Research to find placement options for clients.
Additional Minimum Qualifications
A. Rule 25 training needed within six months of hire.
Preferred Qualifications
A. Licensed Alcohol and Drug Counselor License (LADC) upon hire.
SOCIAL WORKER LICENSING UNIT - ADDENDUM
A. Monitor license timelines for current providers and training timelines for current providers.
B. Provide support and oversight for licensed child foster care and family childcare providers.
C. Prepare and review paperwork for licensing renewal.
D. Complete childcare assistance visits and background checks.
E. Coordinate background studies through Net Study systems for all household members over 13 and substitute caregivers.
F. Provide support with investigations of complaints and violations of licensing statute.
G. Ensure MN statute and rules governing licensed foster care.
H. Assist providers through initial licensing process and investigate complaints and violations of licensing statute.
I. Coordinate visits and inspections to maintain licenses.
J. Complete investigations of foster care and family childcare sites.
K. Follow up with DHS to ensure procedures are followed to implement consequences for licensing violations.
L. Coordinate and implement strategies for recruitment and appreciation of foster providers.
M. Licensors participate and/ or facilitate regional and state resource/ consultation meetings.
Additional Minimum Qualifications
A. Must complete Fire Marshall inspection training every four years.
B. Must complete DHS required licensor trainings within first six months of employment.
C. Licensors must also accrue a minimum of six licensing training hours annually.
SOCIAL WORKER ADOPTION / GUARDIANSHIP - ADDENDUM
A. To locate and support adoptive parents.
B. Advise and consult with CP case managers on permanency issues.
C. Support parents through Report to Court post.
D. Complete parent home studies through ICPC and supervising placement if approved.
Additional Minimum Qualifications
A. DHS Foundations training within six months of hire.
B. DHS permanency trainings within one year of hire.
C. PACC adoption training within two years of hire.
