Under supervision and within State, Federal, and County guidelines; Adult Mental Health Assessment Social Workers are responsible for determining eligibility for Rule 79 Mental Health Targeted Case Management (MH-TCM) and providing MH-TCM services. This position involves coordination and collaboration with clients, detox, hospitals, community-based providers and other County departments. The social worker may be cross-trained to provide pre-petition screenings, and services to the jail. Duties & Responsibilities: These examples do not include all possible tasks in this work and do not limit the assignment of related tasks in any position of this classification. Regular attendance according to the position\'s management approved work schedule is required for all positions.


  • Maintain and apply knowledge of relevant policies, procedures, and statutes and professional practice.
  • Assist individuals in obtaining necessary evaluations to determine eligibility for services.
  • Maintain appropriate engagement and boundaries with clients and colleagues. ESSENTIAL JOB FUNCTIONS: Duties 1-14 are essential functions. Knowledge, Skills & Abilities and Work Environment REQUIRED: Able to effectively use, learn and integrate various software applications/programs into practice. Knowledge of human development, family dynamics and systems, including crisis management and cultural competency. Interviewing, negotiation and listening skills. Ability to communicate effectively both orally and in writing. Ability to work independently and prioritize responsibilities. Demonstrates an understanding of and respect for the diversity of customers, co-workers, and supervisors, including individuals with a disability or whose first language may be other than English. Ability to engage with and interact with persons (being served and co-workers) effectively. Ability to gather information and solve problems effectively. DEMONSTRATE KNOWLEDGE GAINED WITHIN SIX MONTHS: Knowledge of community resources and funding sources. Knowledge of medical diagnoses, medications and basic medical terminology, as well as medical insurance processes and procedures. Knowledge of statutes, rules and policies pertaining to social work and the ability to research the same. WORK ENVIRONMENT: Work is via telework, in-person work performed in a County office building, and on visits to client\'s homes. Some travel is required. Lifting requirement of up to 20 pounds on an occasional basis. Exposure to intense noise and inadequate lighting may occur on visits to client\'s homes. Equipment used may include, but is not limited to, computer, printer, telephone, calculator, and copy and fax machines. SELECTION PROCESS: The examination/selection process for this classification will consist of a rating of your training and experience from the application materials submitted. The top scoring candidates will be forwarded to the hiring department for further consideration.
  • Represent Dakota County in court, as needed.
  • Develop and implement individualized person-centered plans consistent with DHS requirements.
  • Monitor and document progress on case plan and/or court orders.
  • Complete agency, state, and federal required documentation as related to client activity according to mandated timelines.
  • Provide adult services case management for eligible recipients.
  • Consult with individuals, families, as well as other agency staff and professionals in the community.
  • Assess an individual\'s current need for treatment, community support and services.
  • Assess mental health and substance use treatment needs, collaborate and coordinate with other community services providers around need and level of services.
  • Demonstrates a commitment to support cultural diversity County wide, actively supports the County\'s inclusion, diversity and equity initiatives, and works to address the needs of diverse and underserved communities.
  • Provide other case management functions as assigned
  • Assess and determine eligibility for Rule 79 MH-TCM and provide short-term MH-TCM.
