Social Worker – Full-Time, $ 18.00/Hr-$ 25.00/Hr

Summary

Under the supervision of the Administrator:

• Accesses / coordinates appropriate treatment and services for resident psychosocial adjustment difficulties and effectively communicates with team members, residents, resident families, legal representatives, and community resources accordingly
• Ensures that the corporate / care center social services policies and procedures for pre-admission, admission, advocacy, direct / indirect counseling, discharge, and community liaison services are in compliance and implemented in a timely and efficient manner
• Utilizing knowledge of the casework service process and employing associated techniques, [the Social Worker] interviews, counsels, assesses, and intervenes (as needed), in order to affect positive change in the elderly / disabled resident's emotional, behavioral, and psychosocial functioning

Qualifications / Requirements

• Bachelor's degree in social work or human services field, including but not limited to: sociology, special education, rehabilitation counseling, or psychology
• Must be knowledgeable of community resources and support services
• Must have a minimum of one (1) year of supervisory social work / human services experience in a healthcare setting, working directly with individuals similar to those who reside in our communities
• Per the Centers for Medicare and Medicaid Services (CMS), COVID-19 vaccination is required, with exceptions for medical and religious accommodations when reasonable

Additional Information

Schedule: approx. forty (40) hours/wk

Base Wage: $18.00/hr, Negotiable (DOE)

Years of Experience Incentive Pay: $0.15/per year, up to ten (10) years
