Job Description
Job Summary

Under general direction, is responsible for performing complex case management services that support the Neighborhood and Housing Services, including housing relocation and tenant/landlord counseling. Exercises functional supervision.
This position is an "unclassified" position and has no disciplinary appeal rights to the Municipal Civil Service Commission. This position is likewise subject to at-will employment status and can be terminated at any time for any reason.

8:00 a.m. - 5:00 p.m., Monday - Friday; hybrid office/remote work options available

Essential Job Functions

  • Maintain excellent case notes and client documentation in a data management system.
  • Engage in community outreach events to provide housing counseling to community residents.
  • Conducts home visits and/or classroom visits to connect individuals and families to non-housing related services, such as medical, dental, nutritional, and mental health, and to educate on a variety of topics that may include child growth and social-emotional development, parenting skills, child abuse prevention, guidance and discipline, disabilities, family literacy, and community advocacy. Trains and coaches staff on best practices in child and family case management.
  • Coordinates social service programs by providing high-needs individuals and families in crisis with appropriate community resources and linkages, such as food, housing, clothing, transportation, and education.
  • Assists clients with goal planning and action plans to link them with resources and skills needed to meet their goals.
  • Performs related duties and fulfills responsibilities as required.
  • Develops and implements transition activities for families and children going through a social service program.
  • Consults with departments or divisions to improve productivity and delivery of services.
  • Meet all HUD requirements and follow all standards and procedures required by a HUD Certified Housing Counselor.
  • Performs intensive case management services, including screening, assessments, and referrals to individuals and families with the highest needs such as those related to individuals and families experiencing trauma, exposure to domestic violence and other at-risk factors impacting the well-being of children and families.
  • Collects data, maintains records, and supports compliance in all facets of applicable performance standards and state regulations.
  • Connect residents to emergency assistance and financial counseling to prevent homelessness.
  • Develop reporting documents as needed.
  • Analyzes complex data, policies, practices, systems, procedures and develops written summaries on findings and recommendations.
  • Responsible for providing housing counseling to San Antonio residents.
  • Evaluates and recommends staffing levels to ensure effective utilization of human resources.
  • Conducts home visits to provide a program overview and collect needed program documents.

Job Requirements

  • Valid Class 'C' Texas Driver's License.
  • Three (3) years of experience in social work with individuals and families or related experience.
  • Bachelor's Degree in social work or counseling from an accredited college or university program.
Preferred Qualifications
  • Bilingual in English and Spanish

One year of full-time experience is defined as 30 or more hours worked per week for 12 months. One year of higher education is defined as 30 credit hours completed at an accredited college or university.

  • Knowledge of cultural differences and sensitivity to the implications of culture for service delivery.
