THIS IS A GRANT-FUNDED POSITION.


JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: Under the direct supervision of the Program Administrator, this position exists to support and assist caregivers in determining specific needs and obtaining needed support with projects resources through the network and /or other providers. This position will allow hands on contact with individuals and families through home and office visits.


DUTIES AND RESPONSIBILITIES:

  • Identifies participants eligibility through intakes daily via phone, walk-ins, and -referral from other agencies.
  • Conducts home visits to assess family environment-include monthly and /or quarterly contacts.
  • Develops an individualized support plan - a case plan developed with the caregiver in order for the family to receive appropriate services.
  • Determines specific needs with families.
  • Re-accesses families for eligibility after completion of a year of services through the Relative Caregiver Program.
  • Provides monthly or quarterly face-to-face meetings or phone contact with program participants.
  • Maintains accurate case notes on clients from initial contact up to and ending with case closure; submits accurate monthly reports denoting the number of client contacts, cases opened and closed, and an overview of services provided.
  • Collaborates with outreach agencies to secure additional resources to gain greater family stability.
  • Attends IEP workshops pertaining to family preservation and job training as well as Child and Family Team Meetings (CFTM) with DCS.
  • Ability to facilitate and organize groups and program activities.
  • Performs other related duties as assigned.


MINIMUM REQUIREMENTS:


EDUCATION: Bachelor's Degree in Social Work or related field (TRANSCRIPT REQUIRED)


EXPERIENCE: Two (2) years of experience working with families and children in crisis and diverse families. (Experience with facilitating support groups is preferred).


KNOWLEDGE, SKILLS, AND ABILITIES:

  • Ability to work with families and children in crisis.
  • Ability to adapt to the changing policy and environment that is indicative of the University and state mandates.
  • Ability to be able to identify unsafe environments that might put families and children at risk.
  • Ability to facilitate and manage group sessions.
  • Ability to meet the monthly, quarterly, and yearly goals as set forth by the Program Administrator, University and DCS.
  • Ability to work with others in conjunction with the University, Department of Children's Services, program vendors and families.
  • Knowledge of basic computer skills (Microsoft Word, Power Point, Access, Excel, etc.).
  • Knowledge of local and state community resources and services.
  • Ability to be prompt, detail-oriented, courteous, efficient, and professional.
  • Ability to maintain confidentially and professionalism as it concerns information about staff, families, and program matters.
  • Knowledge of community and social services resources.
  • Ability to perform home visits. Must be able to work closely with families and program team members in a positive manner.
  • Ability to work independently, efficiently and to accomplish both routine and detailed tasks.
  • Ability to be empathic and a good listener.

LICENSE/CERTIFICATION: Valid Driver’s License Required (COPY OF LICENSE/CERTIFICATION REQUIRED)


WORK SCHEDULE: This position frequently works weekends and occasionally work evenings. This position requires travel within Shelby County, with occasional overnight travel.
