Job Description
Essential Duties And Responsibilities
  • Updates team spreadsheets and reports with call outcomes
  • Other tasks that contribute to outreach of patients and providers
  • Make outbound calls to patients and prospective patients
  • Utilizes multiple software systems to identify patients needing outreach
  • Uses company knowledge to inform patients regarding services they are eligible for
  • Able to work under moderate supervision
  • Makes calls to doctor’s offices, nursing homes, and hospitals to retrieve necessary information for referrals
Skills And Qualifications
  • Strong interpersonal skills
  • Good written and verbal communication skills
  • Good computer skills including knowledge of Microsoft Office (i.e Word, Outlook, Excel, etc)
  • Self-motivated
  • Problem Solver
  • Strong organizational skills
