Position: Clinical Transformation Assistant

As a Senior Clinical Transformation Assistant, you will play a pivotal role in our Unified Care team, combining technical expertise with clinical acumen to develop an innovative, team-based chronic disease management program. Utilizing our cutting-edge iHealth devices and the Unified Care platform, you will collaborate with other clinical team members to manage patients with chronic conditions both on-site and remotely.
You will provide direct patient care, following established protocols, and coordinate with the team to achieve our shared goals.
Key Responsibilities
Embrace your tech-savviness and comfortably utilize various internal messaging platforms (Google Hangout, Zoom, Google Spreadsheets, PDFs, printers, etc.). Your patience and ability to explain technology to non-tech savvy individuals, especially our elderly patients, will be instrumental.
Foster effective communication with teammates, providers, and clinic staff.
Assist with direct patient care procedures and related tasks, including patient check-ins, obtaining patient histories, vital sign measurements, chart preparations, and medical examination support.
Maintain accurate documentation in electronic health records, ensuring the highest standards of accuracy and privacy.
Perform specialized procedures, such as setting up iHealth products for patients, adhering to established protocols.
Obtain necessary lab samples and other tests as directed by providers.
Manage clinic inventories and medical supplies, ensuring adequate stock levels.
Educate and advise patients within established guidelines, offering guidance on medical device usage and troubleshooting.
Ensure smooth patient flow according to provider schedules and appointments.
Maintain cleanliness and organization of assigned examination rooms.
Coordinate routine office activities and administrative functions to support clinic operations.
Adhere to safety, environmental, and infection control protocols, prioritizing patient well-being.
Required Skills And Qualifications

A bachelor's degree or higher in Health Science, Public Health, or a related field.
Compensation: $19/hr-$22/hr based on experience.
