The Case Manager I will be responsible for general case manager tasks and duties within the social learning environment. This individual ensures the effective care and treatment of the patients and will assume a lead patient advocate role. This individual will coordinate functions to ensure patients actively involve themselves in activities which will promote acquisition of skills to enhance their ability to function more independently and successfully. Each case manager will possess efficient knowledge and skills to provide adequate services to ensure treatment needs of forensic mental health patients and to assist and consult with staff representing other disciplines in preparation of each patient’s individualized patient centered recovery plan. This individual will be under the clinical supervision of the Chief Social Worker on the program.

Essential Job Functions:
Functions as an effective member of the Treatment Team in the formulation, implementation, and review of comprehensive treatment plans for patients on the unit.
Completes required documentation using information from pertinent clinical evaluation tools and processes maintained within the program.
Provides ongoing case management to include but not limited to town orders, phone cards etc.
Serves as a liaison between patients, their MHA, and members of the patient’s treatment team.
Conducts Didactic classes with assigned patients according to approved level of clinical expertise and the needs of the program.
Actively participates in all necessary audits, evaluations and observations.
Assists with planning and preparation for discharge.
Performs related work as assigned.
Actively participates and/or serves in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
Provides staff coverage when required.

Possess a general knowledge of available community resources and the means to access them effectively.
Demonstrate an ability to read and comprehend clinical assessments, identify pertinent information, and be able to integrate these reports and information into treatment plans.

Initial Selection Criteria:
Associate's degree from an accredited college or university with major course work in social, behavioral, or related field. Related work experience may substitute for the required education for a year for year basis with a maximum substitution of two (2) years.
Excellent written communication skills including the use of proper grammar, spelling, and capitalization is required as evidenced by application.
Work or internship history at NTSH is preferred

Multiple positions on multiple programs on the Vernon Campus of NTSH are available. This position qualifies for Hazard Duty pay of 6.8%.
