Job Description
$2,000 sign on bonus included! Community and Family Resources serves candidates with a variety of mental health and substance use concerns. We are seeking candidates for a Case Manager to provide services to our clients on the TIEH (Treatment for Individuals Experiencing Homelessness) grant and who reside in our transitional housing facility. Services include providing resources and recovery support.
Essential Duties And Responsibilities
  • Continue to work with the person served to continue engagement in SUD (Substance Use Disorder) Treatment or other Co-Occurring Disorder Treatment.
  • Work collaboratively with other agencies and community stakeholders that serve the homeless population.
  • Provide TIEH services in a language and cultural context that is appropriate to the person served. This may be through the use of a language interpretation service.
  • Provide orientation to newly enrolled candidates regarding Recovery Support Services (RSS) that are available to them through the grant.
  • Participate in evidence based trainings related to grant.
  • Accept or deny applicants for entry into the TIEH Grant, through the use of screening tools.
  • Participate in routine reporting of Department and Government Performance and Result Act (GPRA) data.
  • Provide Recovery Support Services to all enrolled persons served on the grant and maintain a case management approach to keep them engaged in RSS and treatment services.
  • Participate in various project, council, and/or stakeholder meetings such as the grantee meetings, statewide meetings regarding homelessness, monthly provider calls, and others requested by IDPH.

Education And Experience Qualifications
  • A minimum of 1 year of case management or counseling experience is preferred.
  • Bachelors Degree from in a related field is required. Lived experience preferred.
Required Knowledge And Abilities
  • Ability to communicate effectively, regularly, and fluently in English; bilingual (English/Spanish) abilities are preferred but not required.
  • Ability to maintain the confidentiality of client information as mandated by various federal and state laws.
  • Must possess good verbal and written communication skills.
  • Ability to manage potentially traumatic situations including distressed clients, clients’ family members, and/or responsible parties,
  • Due to the variety of changing tasks, the individual will need to be flexible and assist in other duties as assigned
  • Ability to establish good working relationships with clients and staff.
  • Must possess good verbal and written communication skills.
  • Ability to complete written reports and form completion in a timely manner and accurate manner as needed.
  • The staff member must be able to work in a fast-paced environment with demonstrated ability to manage and prioritize multiple, competing tasks and demands and to seek supervisory assistance as appropriate.
  • Ability to speak effectively before groups of clients or agency employees.
  • Knowledge of 42CFR and how it applies to Community & Family Resources.
  • Ability to discern and report relevant client information to clinical staff for appropriate management of client care.
  • Must possess basic computer skills and the ability to document client information, as requested or required into the computer system.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Knowledge of HIPAA rules and regulations.
  • Ability to prepare regular and special written and oral reports.
  • Ability to prepare clear and concise records.
  • Ability to move between agency buildings, from office to office, to enable conferring with other staff members.
  • Ability to analyze problems presented and to develop and implement effective courses of action.
