Job Description

Pay: hourly rate equivalent of $45,000-$47,000 per year. QIDPs are hourly and do get overtime pay. Located on State Street in Rockford!

Schedule: Primarily Monday through Friday with some weekend and evening time when on-call. Work from home two days a week. High degree of flexibility in setting your office hours. Assigned a laptop to make remote work easier.

On-call: The QIDPs share a medical emergencies on-call with other managers (NOT on-call to cover DSP shifts).

Position Summary: The QIDP works with the people served to help them create a life of their choosing and to help them advance toward achievement of their personal goals and desired outcomes. The QIDP facilitates a Community Support Team (CST) for each person served to craft and monitor a service plan based on the wants and needs of the person served and their support network and serves as a mentor to all members of the team toward that end, as needed.

Basic Responsibilities Include:

  • Being the main point of contact for families served, schools and day training programs.
  • Visiting people served in their homes.
  • Coordinate referrals of people seeking services, admission, and discharges of people served.
  • Helping people served and their families reach their personal goals.
  • Developing implementation strategies for people served.

Things our QIDPs DO NOT have to do:

  • QIDP Notes.
  • Supervisie DSPs.
  • Deal with SSI, Social Security or Medicaid/Medicare.
  • Create or manage DSP schedules. (Except when on call)


Basic Qualifications:

  • Must have a bachelor's degree in a human services related field.
  • Must have one year experience with people with developmental disabilities and/or autism spectrum disorder. This can be work experience, personal experience, or a combination of both.

Acceptable degrees are:

  • Bachelor level degrees where there is sufficient coursework (at least 30 semester hours) in human services areas.
  • Human skill development: education counseling, human development, humans and their cultural behavior (e.g., anthropology), etc.
  • Human behavior: social work, anatomy, communicative disorders, applied behavioral studies, counseling, gerontology, kinesiology, physiology, health education, elementary education, etc.
