Job Description

Job Summary
Care Connections, a division of Molina Healthcare, seeks Outreach Agents to join its growing team which will contribute to the company mission to bring quality health care to the underserved. The Care Connections Representative primary focus is to conduct outbound calls to Molina members and schedule appointments with our Nurse Practitioners across 14 states. As part of Molina's benefit package, the appointments are for preventive care services delivered in the home, community, and nursing facility settings. This is a special opportunity to make a positive difference in the lives of vulnerable people.
Knowledge/Skills/Abilities
  • Demonstrates personal responsibility and accountability by meeting attendance and schedule adherence expectations
  • Bilingual (English/Spanish preferred).
  • Ability to multi-task, set priorities and manage time effectively
  • Perform timely documentation in electronic medical record
  • Demonstrate positive working relationships with peers and effectively manage conflict
  • Attention to detail
  • Problem solving skills
  • Strong phone and verbal communication skills along with active listening
  • Empathy/passion for working with senior, disabled, low income populations
  • Ability to abide by Molina's policies and procedures
  • Maintain confidentiality and comply with Health Insurance Portability and Accountability Act (HIPAA)
  • Show flexibility in meeting changing performance objectives consistent with Molina and department objectives
  • Understanding and striving to meet or exceed call center metrics while providing excellent consistent customer service
Job Qualifications
Required Education
  • Associate Degree or equivalent combination of education and experience
  • High School Diploma or GED

  • Experience doing outbound appointment setting or similar services
  • Excellent customer service, active listening, and verbal and written communication skills, professional phone voice.
  • Customer Service/Call Center experience in Healthcare or equivalent related
  • Working knowledge of Microsoft Office (Outlook, Word, Excel) or other comparable software
Pay Range: $12.32 - $24.02 an hour*
