Family Health Advocate Jobs

Provides family focused, solution-based risk assessments of families and conducts home studies.

Initiates alternative out-of-home placements such as in the home of a relative or in foster care for children needing care.

Completes individual service plans and group care packets, as needed.

Writes and reviews progress reports and status change reports. Document all contact and services.

Monitors visits between children and their families when appropriate. Ensure monthly contact with placement families and individually with the children.

Diligently pursues active efforts with parents to ensure rapid and viable reunification.

Builds trusting relationships with parents to provide professional case management, prevention and early intervention services.

PRINCIPAL DUTIES AND RESPONSIBILITIES(Not All Inclusive)

Monitors child's attendance and works closely with parents to identify obstacles and problem solve with them to establish regular attendance.

Actively promotes Head Start in the community by participating in recruitment efforts and activities.

Supports parents with transition into and out of ELE services

Demonstrates understanding of the child abuse/neglect laws and reports suspected child abuse to the Family & Community Services Director.

Ability to interpret verbal and non-verbal communication; demonstrate situational awareness, compassion and empathetic listening skills

Previous experience effectively advocating, collaborating and negotiating with medical professionals to solve complex problems

Ability to demonstrate assertive responsiveness to changing clinical and social/emotional dynamics, and to drive process

Ability to demonstrate a high level of accountability with internal and external partners

Conduct ongoing monitoring, tracking, follow-up, and analysis of enrollment and attendance data, and produce regular reports for management meetings.

Responsible for tracking and documenting health events in the program’s data management system.

Support the referral process for children with possible special needs and works collaboratively with the Education/Disabilities Team, teachers, and parents.

Education: Certification or Credential in social work, family services, human services, or in a related field.

Working knowledge of local community resources and health requirements for children, zero to three years of age.

Experience: Work in a Head Start of Early Head Start program and knowledge of the Office of Head Start Performance Standards.

Knowledge / Skills / Abilities:

Minimum Educational Requirements Associate Degree

Provide materials, activities and training to parents and families to help them effectively support their children at home and at school

Help parents and families to understand curriculum standards and assessments.

Participates in the school's School Leadership Team (SLT)

Organizes a flexible number of curriculum-based programs and activities for family engagement (quarterly)

Knowledge of best practices in delivery quality early care and education with family engagement

Knowledge of working with children and families through a multi-generational approach

Experience providing services to children and families in diverse communities

Recruitment, intake and enrollment, eligibility and attendance monitoring

Support collection and maintenance health and immunization records

Develop and conduct parent workshops

Read current literature to maintain and develop knowledge of issues affecting families and service and in the delivery of care

3 years' experience in a health care clinical setting required

Coordinate employee training on patient/family complaints and grievances

Identify areas needing additional training in policies and procedures related to complaints and grievances

Ensure that issues and concerns raised by patients and their families are handled in an effective and expedient manner

Assume on-call duties after hours, on weekends, and on holidays on a rotating basis

Previous case management experience working with children and families

Conduct crisis case management and intensive case management, as appropriate

Assist clients in completing applications for Texas Crime Victim’s Compensation Benefits

Possess a working knowledge of child abuse issues

Salary range: 47,000 plus depending on years of experience

Provide consistent follow-up with clients, often as a liaison between families and partner agencies

2 – 4 years of experience related to customer advocacy.

Documents member activities and progress notes in appropriate systems and provides reports to management

Assists clients with obtaining and completing appropriate application forms for various benefits and services

Experience with at-risk or gang-impacted youth and/or adults in a clinical setting

Working knowledge of the criminal justice system

Practical knowledge of legal and health resources available in community

Practical knowledge of community violence and its impact on human development

Must possess knowledge of principles of case management

Must possess knowledge of community services and human service agencies

Must possess ability to manage time effectively

Provide leadership that supports Public Health accreditation, emphasizing performance management processes and measurements.

Supervise, manage, and monitor the operations of a Family Health Home Visiting work unit and associated programs.
