Job Description
MISSION STATEMENT


Homeboy Industries provides hope, training, and support to formerly gang-involved and recently incarcerated people, allowing them to redirect their lives and become contributing members of our community.


Job Summary


The Legal Clinic Coordinator and Health Advocate will serve as a liaison between the legal, wellness and case management departments. The position will plan, oversee and implement all legal clinics serving trainees and community members. As Health Advocate, the advocate will work closely with case managers and the wellness team to ensure that all trainees are connected to appropriate health resources.


Duties & Responsibilities


  • Actively participates as part of provider practice Interdisciplinary Team Meetings providing updates on member needs and progress
  • Accompanies trainees in accessing medical, vision, dental, and mental health care
  • Oversees planning and implementation of all legal clinics and partnerships including but not limited to: community client expungement clinics, family law clinics, public defender clinics, mediation clinics, and housing clinics
  • Documents member activities and progress notes in appropriate systems and provides reports to management
  • Connects with legal providers and advocates to disseminate relevant information and resources to community
  • Determines social determinants of health concerns/ gaps, develops and documents a plan to address moderate social and health disparities
  • Performs other duties as assigned
  • Identifies gaps in community resources and medical systems and supports the implementation of new solutions or services to close identified gaps
  • Assists clients with obtaining and completing appropriate application forms for various benefits and services
  • Assists trainees in understanding health options available under insurance plans and beyond
  • Assists with onboarding and training of legal team volunteers and interns
  • Establishes relationships with and partners with community resources, health plans and providers by participating in local community engagement activities with local agencies e.g. faith based organizations, community centers, government agencies, parks, recreation centers and schools


Qualifications


  • Ability to relate sensitively to persons of diverse ethnic and socio-economic backgrounds
  • Mission-driven person with strong work ethic
  • Able to meet program goals and deadlines.
  • Able to multi-task and work in a timely and accurate manner
  • Experience interfacing with other service entities such as DMH, DPSS, Probation, DCFS, LAUSD, Hospitals, Domestic Violence crisis centers, Dual Diagnosis treatment programs, etc.
  • Expertise in cross-cultural and culturally empathic practices.
  • Familiarity with trauma-informed care
  • Experience with at-risk or gang-impacted youth and/or adults in a clinical setting


SKILLS AND KNOWLEDGE REQUIRED:


  • Computer skills
  • Strong communication and organizational skills
  • Bilingual, Spanish-speaking a plus
  • Practical knowledge of legal and health resources available in community
  • Collaborative ethic and eagerness to build partnerships with other providers
  • Working knowledge of the criminal justice system
  • Practical knowledge of community violence and its impact on human development


WORK ENVIRONMENT


  • Combination of field and office environment.
  • Work regularly in a community that is exposed to gang activity, substance use, homelessness, and high unemployment rates.
  • On occasion walk or drive to different local sites throughout the day.
  • This is NOT a work-from-home position
  • Working in busy and loud environments.


PAY RATE


$24-$26/hr. DOE
