Job Description

The support coordinator is responsible for the overall management of his/her assigned cases. Support coordination is a Division-Funded service that assists individuals in gaining access to the needed program and State Plan services, as well as needed medical, social educational, and other services.

Responsibilities

The Support Coordinator manages Support Coordination services for each individual by performing 4 general functions: individual discovery, plan development, coordination of services, and monitoring.

Scheduling and facilitating planning team meetings in collaboration with individuals and writing the PCPT and ISP, and distributing the ISP. Complete PCPT and ISP by 30 days from the date the individual is enrolled into the Support Program (annually thereafter)

Monitoring and following up to ensure delivery of quality services and ensure services are provided in a safe manager, in full consideration of individual rights.

Performs all other duties as assigned.

Qualifications

The positions required a Bachelor's degree or higher in any field and a minimum of 1 year of professional experience working with adults (21 or ) older) with intellectual/developmental disabilities, special education mental health counseling guidance social work, or health and rehabilitative services. Computer skills are a must, complete the trainings required by DDD before rendering services and successfully pass a Federal Criminal Background and Central Registry check at the point of hire. n
