SHIFT (Hours of Work, Days Off): Monday – Friday, variable schedule between 7:00 a.m. to 6:00 p.m., travel.

The Iowa Department of Public Health (IDPH) and Department of Human Services (DHS) merged on July 1, 2022, to form one agency, the Iowa Department of Health and Human Services (HHS).

DUTIES: The person in this position will work with individuals diagnosed with Intellectual Disabilities (ID), Developmental Disabilities (DD), and Brain Injury (BI), and individuals utilizing the Health and Disability (HD), Physical Disability (PD), Elderly and AIDS/HIV waivers. This position will act as an advocate to help people with the above disabilities identify service needs, obtaining services to meet those needs, and evaluating progress in the provision of those services. Included in these duties are development of the individual care plan, service coordination across agencies, resource acquisition, assisting consumers and/or guardians in navigating the service system, securing crisis services, finding new services and tracking outcomes to ensure the consumer’s needs are met in a way that allows them to remain independent in their community and participate in decisions regarding their service plan. Case Managers do not provide direct service to consumers. Case Managers are expected to meet a billable unit expectations. Travel is required.

Competencies include: Knowledge of case work methods, knowledge of the target populations, interviewing skills, team building and team facilitation skills, effective customer service skills, ability to evaluate situations and make correct decisions; ability to correctly interpret laws, rules, regulations, policies, and procedures; ability to express ideas effectively, orally and in writing; ability to drive a motor vehicle. Experience using Microsoft Office programs and/or electronic medical records preferred.

Minimum qualifications are: Graduation from an accredited four-year college or university.

In Addition, You Must Meet The Following Selective

920 Case Management - Bachelor’s Degree with at least 30 semester hours including: Behavioral Science, Education, Health Care, Human Services Administration, Social Sciences and; applicant must also possess 1 year experience in delivery of human services in the combination of: Chronic mental illness, Developmental disabilities, and Intellectual disabilities; OR applicant must have Iowa license to practice as a registered nurse and the equivalent of 3 years nursing or human services experience.

Minimum Qualification Requirements

Graduation from an accredited four-year college or university;

OR

the equivalent of four years of full-time technical work experience involving direct contact with people in overcoming their social, economic, psychological, or health problems;

OR

an equivalent combination of education and experience substituting the equivalent of one year of full-time qualifying work experience for one year (thirty semester or equivalent hours) of the required education to a maximum substitution of four years.
