About PACE


Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a new non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

One Team One Mission


Position Summary


The Scheduling Assistant/MA will coordinate and schedule appointments for all participants of Gary and Mary West PACE. This position will prepare schedules according to the participant needs and the participant's care plan designed by the interdisciplinary team. The Scheduler will work closely with the transportation department, PACE participants, caregivers, and the Center staff.


Essential Functions


  • Create and maintain clinic schedule for PACE care team according to participant care needs.
  • Work closely with PACE leaders to ensure accuracy of all information.
  • Create electronic health record (HER) authorization, transportation appointment, escort appointment, etc.
  • Coordinate participant's medical appointment schedules with Adult Day Health and interdisciplinary team.
  • Effectively coordinate and communicate with all departments within PACE that are involved with the participant appointments.
  • Be flexible in schedule of hours worked.
  • Call participants and/or caregivers and remind them of next day appointment.
  • Maintain strict confidentiality of all participant information.
  • Record all scheduling changes and updates daily.
  • Attend and participate in all staff meetings, in-services, projects, and committees as assigned.
  • Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
  • Input return visit requests and semi-annual assessments into schedule.
  • Interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
  • Adhere to and support the center's policies, practices, and procedures.
  • Observe each participant for any change in physical, mental, emotional, and social functioning. Report such changes to the licensed nurse.
  • Serve as a back-up Medical Assistant as needed by performing clinical activities that encompass all patient needs related to the safe and effective delivery of patient care.
  • Participate in continuing education classes and any required staff and training meetings.
  • Ensure participant education has been completed by nursing prior to appointments.


Education And Certification


  • Certified Phlebotomy Technician I (CPT I), preferred.
  • High School diploma or GED required.
  • Graduate of an accredited Medical Assistant training program required.


Skills & Experience


  • Bilingual is required for this position.
  • Minimum of one (1) year of documented experience working with a frail or elderly population required.
  • Ability to follow through with instructions and directions provided by supervisor, RN, QAPI coordinator, primary care physician, and/or medical director.
  • Must have a personal vehicle to use on a daily basis and provide and maintain proof of auto insurance.
  • Skilled in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants.
  • Experience in a long-term care facility or with a community-based geriatric program preferred.
  • Ability to work effectively and harmoniously as part of an interdisciplinary team, the elderly, providers of services, and peers.
  • Well organized, dependable, flexible, and resourceful.
  • Must be able to demonstrate proficiency of Microsoft computer applications. Experience working with Electronic Medical Record system (EMR) preferred.
  • Ability to utilize appropriate safety measures when providing care.


Medical Clearance


Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Job offers are contingent upon a successful pre-employment drug screen, background check, and physical assessment.
