Position Summary


The Resource Coordinator/Medical Assistant/Phlebotomist (RC/MA/PB) position serves a multifaceted role within the Homeward Bound/Palliative Care (HWB/PC) team. The RC/MA/PB provides administrative support as well as support to patients under the direction of the Interdisciplinary Team (IDT) including the RN Care Manager, providers, and Senior Program Leader, using a team approach to patient care. Strong administrative, computer and phone support skills are required along with a desire and aptitude to assist providers and staff with direct patient care, including an active certification to provide phlebotomy services in patient’s homes, as needed. The RC/MA/PB will work closely with community partners, develop and coordinate access to resources for patients/families and assist with transitions of care from various settings. Additionally, this position will provide excellent patient care in a compassionate, caring, culturally and ethnically sensitive manner that promotes the organization’s mission, vision, and core values.


Schedule: M-F 8-4:30


Resource Coordinator


Essential Duties and Responsibilities:


  • Coordinate with HWB/PC team for scheduling of patients for home visits, virtual visits, group Grandpad visits and phlebotomy services
  • Assist patients/families with access to resources to address any barriers to care, help with coordination of services and work with HWB team to ensure patient receives such services
  • Lead the IDT meetings, and accountable for documenting and coordinating with external partners to follow patients through transitions of care.
  • Answer phone calls, create patient cases, and communicate with all HWB/PC team members, external partners, and other HHHN staff
  • Work with discharge liaisons at local nursing homes and hospitals during transitions of care to assist with coordination of services, setup home visits, and inform the IDT team of transition plans


Medical Assistant


  • Maintain annual competencies and required continuing education for the Medical Assistant Certification
  • Create and promote adherence to a care plan developed in coordination with the patient, primary care provider, RN Care Manager, and family/caregiver(s)
  • Coach patients pertaining to red flags and follow plan of care directed by Provider and/or RN Care Manager
  • Increase patient’s ability for self-management and shared decision-making
  • Review the patient’s health history to include pertinent preventative, social and past medical conditions; use available resources for patient education and follow-up care per provider order
  • Enthusiastically serves on various departmental initiatives that support the Mission, Vision and Core Values
  • Facilitate patient access to appropriate medical and specialty providers, including assisting with coordinating transportation
  • Follow-up on all Homeward Bound (HWB) patients that have been discharged from the emergency department, hospital, or rehabilitation/nursing home within 48 hours via phone, telehealth or home visit when necessary
  • During patient encounter, document chief complaint, obtain a height and weight, temperature, pulse, blood pressure, respirations, and check pain level. Obtain peak flow and pulse oximetry when indicated during any home visit
  • Provide medication reconciliation and determine need for medication renewals, prep orders for provider approval during telehealth or home visit encounter
  • Adhere to standards, policies and procedures for diagnostic and point of care testing, including specimen collection, quality assurance, documentation in the electronic medical record and patient education. This may include transportation of biological or other medical materials for IDT team members
  • Complete pre-visit planning for all patients prior to appointment
  • Perform additional duties and special projects as assigned
  • The Hudson Headwaters Medical Assistant (MA) will adhere to and uphold all necessary training that is required to meet the National Healthcareer Association Certification requirements and/or complete Hudson Headwaters Medical Assistant training course.
  • Lead and operationally oversee Grandpad deployment, patient programing and focus on improving socialization through the Grandpad for improvement in patient outcomes


Phlebotomy


  • Maintain credentialing, training, and any necessary policy requirements to perform phlebotomy services
  • Order necessary supplies to perform all necessary home lab draws or specimen collections
  • Willingness, if not already certified in Phlebotomy, to provide patient blood draws or specimen collection in the patient’s home
  • Coordinate with Glens Falls Hospital on any lab needs or requirements for necessary testing and results
  • Coordinate and schedule home draws in coordination with the IDT team members, partner organizations and patient needs


Qualifications


The requirements listed below are representative of the knowledge, skills, and ability to perform the essential functions:


  • Knowledge of diagnosis codes needed for specific testing to ensure correct billing is submitted
  • Strong verbal and written communication skills
  • 6 plus years of phlebotomy experience with ability to adapt to different environments
  • Must be self-directed, detail oriented, motivated, and able to work independently as well as in a team atmosphere
  • Knowledge of practice management system, electronic medical record and all interfaces related to laboratory reporting and test submissions
  • Demonstrated computer competencies including electronic medical record and/or practice management experience
  • Prior experience with community resources and knowledge of medical terminology beneficial
  • Medical Assistant Certification required through National Healthcareer Association (NHA) completed within a year of employment from date of hire (Employer paid) OR complete Hudson Headwaters Medical Assistant training course within first six months of employment
  • High School diploma at minimum with 8 years or more of health care experience; prior experience in ambulatory care preferred
  • Current BLS certification required
  • Knowledge of general phlebotomy practices and related specimen collection/preparation processes
  • Knowledge of lab values and ability to discern when it is necessary to alert staff of an abnormal (high or low) value requiring immediate attention to ensure patient safely and optimum care
