Position Overview


The position of Logistics Coordinator is a Supervisor position. They are responsible for the coordination of transportation services for the Children at our Foster Care Program, Family Place Visitation Center. They will also facilitate visitation across the entire visitation services collaborative. Logistics Coordinators will maintain visitation room assignment schedules for the families we served. They Supervise a team of 5 Transporters as they transport children in a safe manner, monitor their time and arrange for other job duties as necessary.

  • Complete performance evaluations for Transporters according to agency time-lines.

Overview of the role of the Youth and Family Rehabilitation Professional


The role of a Rehabilitation Professional is one of multi-components that can include interventions such as case management as well as other functional supports. In this role you will provide services that are goal-directed and include solution-focused interventions. The goals and solutions are intended to address challenges associated with a behavioral health need and achieve identified goals or objectives as set forth in the child’s treatment plan. Any activities that you would provide are intended to stabilize the child/youth in the home and natural environment.


Position Details Currently, there is one position available. It is Part-time, up to 20 hours per week, however, it will fluctuate because it is based on client need.


Hourly Wage $19.00/hour depending on your education and experience.

Essential duties and responsibilities


  • Assisting families to seek and utilize supports to restore stability and maintain functioning and prevent the severity of impact of current and/or future crisis situations and increase safety in the home and community.
  • Responsible for maintaining timely completion of documentation and visit productivity to meet required fee for service billable units.
  • Coordinate services to address identified rehabilitative needs within an ongoing comprehensive treatment plan including collaboration with other treatment providers in according with care planning.
  • Demonstrate cultural competence and trauma-informed practice, as well as integrate understanding into daily responsibilities and participate in meeting the objectives of The Salvation Army, Syracuse Area Services Strategic Plan.
  • Complete Medicaid documentation and billing according to NYS and agency standards and proceed with service without proper documentation for client referrals and recommendations to initiate services.
  • Provide community-based services to children and families who may have difficulty engaging in formal office settings but can benefit from home and/or community based rehabilitative and restorative services.
  • Engage with child/youth and family/caregiver in ways that support the everyday application of treatment methods and goals.
  • Provide services within a variety of permissible settings including, but not limited to, community locations where the child/youth lives, works, attends school, engages in services, and/or socializes. Services may also be delivered at community-based agency facilities.
  • Services may include intensive case management and planning services to avoid crisis as appropriate.
  • Provide education, planning, and rehabilitation to help identify and address necessary goal areas as directed by the treatment plan or plan of care.
  • Report and communicate with direct supervisor regularly within supervision and as necessary as possible to ensure client needs are met and program requirements are fulfilled.
  • Complete all necessary agency and Medicaid required trainings.


Qualifications


Education


  • Bachelor's degree from a four-year college is required.


Experience


  • NYS Driver License that meets The Salvation Army insurance requirements.
  • Two years of applicable experience with at-risk children’s populations including mental health, foster care, child welfare, and/or juvenile justice.
  • Must have access to private transportation
