Seva Hospice is a locally owned and operated company of experienced hospice providers with a desire to serve the communities we live and work in. Due to growth, we are looking for an Intake Coordinator to join our dynamic team to support our clinical team. This will be a part time position, Thursday through Sunday to provide weekend coverage. The Intake Coordinator is responsible for coordinating all aspects of the patient intake process including maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. Collaborates with the Director of Patient Care Services and Account Executives to ensure timely and appropriate admissions.
ESSENTIAL RESPONSIBILITIES
1. Coordinates daily patient referral and intake operations with DPCS and Account Executives.
2. Enters patient information into EMR, including detailed referral information as it relates to patient transition to home care.
3. Maintains patient confidentiality and protects operations by monitoring confidential information.
4. Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
5. Maintains positive working relationships with current and potential referral sources.
6. Ensures seamless transition of patients to home care by providing patient education and preparation for home care and coordination of care with multiple service providers.
7. Participates in insurance verification and authorization processes.
8. Assists in assessing DME needs and pacing orders for approved DME.
9. Gathers, collates, and reports referral statistics including key customer referral trends.
10. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by the organization.
POSITION QUALIFICATIONS
1. At least two (2) years’ experience in health care management, preferably in home care or hospice operations.
2. Demonstrated ability to maintain performance under a fast-paced, heavy work environment.
3. Ability to deal tactfully with customers and the community.
4. Demonstrates good communications, negotiation, and public relations skills.
5. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
