AbleLight is currently seeking an Intake Coordinator. In this role, the primary responsibility is to triage all incoming referrals via phone, inbox/email, and agency portal, when applicable, utilizing our standard intake process and criteria to identify clients that are eligible for our residential service settings.
The Intake Coordinator accomplishes their work by facilitating a collaborative multidisciplinary team that may include operations, nurses, and leadership and key external stakeholders. This individual will capture all referral activity across the organization in our standard template and system. In addition, will request and review all relevant information working with referral agencies, providers, prospective family, and other external partners to prepare the standard review packet for operations team review and decision making.
The Intake Coordinator will coordinate with the local team, referral agency and the prospect family. The coordinator will be responsible for communicating the review process and the decision once approved by the local Operations team. When necessary, they will prepare any additional review for a secondary governance review.
Requirements:
- 2 years related experience with 2+ years of experience coordinating referrals in health care or social services experience with IDD (Intellectual and Developmental Disabilities) services beneficial.
- Curious learner who leverages research and technology to the fullest potential.
- Must have a high school diploma.
- Exceptional collaboration, customer services, and listening skills are key for internal and external relationships.
- Less than 10% travel across the markets based on the needs of those we support, our employees and strategic growth.
- Ability to utilize a standard set of operating practices, ensures a high degree of accuracy and timeliness with a record of accomplishment of making quality and continuous improvement recommendations are essential.
- Ability to work effectively in a remote environment.

Responsibilities:
- Maintains strong collaborative working relationships with all internal and external stakeholders involved in the referral and intake process.
- In complex cases that require special attention, appropriately escalate to the “admissions committee” ensuring the best outcome for the client and AbleLight.
- Works closely and collaboratively with local leadership to ensure the data they provide supports operational needs.
- Ensures data accuracy by completing timely input and regular audits.
- Keeps up to date on all state requirements around the referral process and ensures our processes and documents meet all requirements of local, state, and federal regulations.
- This role is responsible for utilizing AbleLight’s standard intake process and systems for new clients in our three residential service settings (group homes, host homes, independent/supported living services).
- Contact referents and/or family members to confirm the prospect family interest and needs and any other pertinent information required for assessment including demographic and insurance information.
- Ensures all data for the admissions packet is accurate and presented timely presented to the local leadership for review and decision making. This will be inclusive of a recommendation based upon the needs of the individuals we serve, and knowledge gained from local leadership.
- Full accountability for ensuring that all referrals are captured in our system using our standard system and templates in the established timely manner.
- Knowledge of all AbleLight’s programs and services in the markets and the ability to explain to patients / families / referents.
- Will meet with local teams to create alignment and review status of active/open referrals to ensure data is accurate in the system for local leadership to ensure achievement of balanced scorecard metrics and business needs.
