Community Nurse Navigator - Community Health

Salary Range: $52.7700 - $81.7400 /hour

The Clinical Nurse Navigator I is a registered nurse responsible for the seamless coordination of care across the continuum for patients with complex, integration-sensitive diagnoses. The navigator is a patient advocate who coordinates care in conjunction with the physicians from the time of diagnosis or admission through discharge or up to the end of life. Educates patient and family/significant others on the health care experience; implements, coordinates, monitors and evaluates care options, services, and resources. Links family/patient to appropriate resources as needed. Proactive in finding ways to enhance the patient experience clinically and in service in order to provide an exceptional patient experience. The Clinical Nurse Navigator also assists in quality improvement initiatives by gathering, tracking, and researching quality metrics. Monitors outcomes, e.g. patient satisfaction, compliance with the treatment regimen, and loyalty. Interacts with department nurses, and may orient and train incoming Clinical Nurses and Patient Care Assistants. Performs other duties as assigned.
