Job Description
:

The Macon County Mental Health Board is seeking a motivated, self-starting individual for the position of Client Care Coordinator for the Juvenile Redeploy Illinois grant program. This is a part-time position that will work 20 hours per week and occasionally up to 30 hours per week as needed. The Client Care Coordinator is responsible for the coordination of case plans for each participating youth of the Macon County Juvenile Redeploy grant program. The Client Care Coordinator will work closely with program youth and the responsible agencies to ensure the best chance of success in the program.

Pay Range- $18.00-$20.00 an hour


Responsibilities:

  • Monitor post-completion success rates for youth at the 12- and 24-month marks
  • Prepare court reports and appear in court, as needed; Communicate with the JRI team to ensure accuracy and understanding of court reports to effectively represent the JRI team to the court; Communicate with the juvenile court judge to determine the preferred process for JRI youth and ensure this process is followed
  • Manage referrals and coordinate intakes to the program
  • Monitor agency alignment with the case plan•
  • Monitor and track progress toward successful completion of the case plan•
  • Collect and analyze data reflecting outcomes for each youth and agency•
  • Other duties as assigned
  • Work with the JRI team to assess each participating youth and develop a case plan
  • Maintain regular contact with youth and respective agencies
  • Schedule and facilitate the Client Review Team (CRT) meetings•
  • Research, recommend, and facilitate professional development training for the JRI team


Requirements:
  • Bachelor’s degree or equivalent combination of education and/or experience
  • Ability to build and maintain relationships and work with youth and community partners
  • Excellent customer service
  • Effective verbal and written communication skills
  • Strong organizational skills

  • Ability to work independently
