Care Team Associate - Remote | Wfh

We are currently seeking a Care Team Associate (CTA) to join our team. As a CTA, you will play a crucial role in supporting the implementation and execution of all Care Management processes. You will work closely with our Case Management staff, including the CM manager, CM Director, and CM Vice President, to ensure that all operational responsibilities are met and that the highest level of customer service is provided to both internal and external customers.
One of the primary responsibilities of this role is to coordinate our weekly Patient Care Conference (PCC) calls and manage our daily census. You will also be responsible for creating authorizations and generating written notifications to providers as required. As a liaison between our clinical team and our customers, you will have the opportunity to provide live outreach to our members, educating them about program benefits and services while also managing member cases.

To be considered for this position, you must have a high school diploma or GED and at least 2 years of customer service experience. Experience with MS Word, Excel, and Outlook is also required. Preferred qualifications include experience working within the healthcare industry and with health insurance, experience working in a hospital or medical clinic setting, experience working in a call center, and a background in clerical or administrative support. Knowledge of ICD-9 and CPT codes is also a plus.
