Prepares and manages daily and weekend schedules.

Prepare and manage the end-of-the-day report and submit it electronically to all required to receive it daily.

Manage and follow hospitalized clients according to Standard Operating Procedures.

Manage electronic and manual faxes received with proper follow-up and distribution as necessary.

Manage and assist with patient phone calls following Standard Operating Procedures.

Must have excellent organizational skills and can work well with others in a diverse and active environment.

At least two years' experience in administrative office functions

Knowledge of computer software, including Microsoft Office applications

Selects appropriate clinician for patient assignment based upon patient needs, location of the patient, and skill of the clinician

Minimum of one year clinical experience, preferably in home health

Minimum of two years of administrative experience in a healthcare environment, preferred

Home health and/or hospice experience

Ensure Oasis visit is scheduled within a 5 day window of end of certification period for each patient.

Moderate knowledge of Microsoft Word, Excel, and Power Point.

Assign therapist and nursing staff to patients once admitted.

Schedule visits in DeVero as ordered by admitting/evaluating staff.

Ensure that staff is seeing patients as ordered and missed visit notes are written when appropriate.

Collects and reviews client needs to support on-going and new scheduling requirements, leverages clinical skillset and scheduling database for support

Comfortable speaking with referral sources and client management

Scheduling and staffing for all skilled care patients

Minimum of 2 years' related experience as highlighted below

Home Health scheduling experience required

Previous experience in a fast paced, high call volume environment

Responds timely to system and/or clinician schedule requests to ensure referrals and ongoing care is scheduled per policy/orders and patient need.

Utilizes system scheduling module, communication tools and reports to ensure timely and well-coordinated patient visits effectively and accurately.

Utilizes other patient information systems to ensure complete referral information attached and available in EMR when appropriate.

Provides timely feedback to leadership when barriers to productivity, staffing needs and or untimely clinician response for communication and documentation.

Monitors assigned reports for compliance with requested scheduling and alerts when scheduling is outside of regulatory or agency policy.

Provides input to management on improving processes and procedures and becoming more cost and time efficient.

Assists the Clinical Manager in establishing and monitoring compliance with quality and operations standards.

Promotes agency through education to prospective and existing patients/clients.

Associate Degree or equivalent years of training or work experience.

One (1) year experience of home healthcare scheduling REQUIRED.

Proven organizational skills and detail orientation.
