Community Health Navigator (FT) 8a-5p (BHCORP) Remote

TENNESSEE (Select facility below), Johnson City, TN

Full-time (scheduled 72 hrs or more per pay period)

8 AM to 5 PM

Effective organizational skills including delegation, managing multiple priorities, time management.

Excellent verbal and written communication skills for groups and individuals of different populations including persons with disabilities and Dementia.

Ability to work collaboratively with community management, internal and external resources/partners and communicate with residents, families, physicians and staff.

Assists the residents with understanding of their specific health plan benefits.

Develops standard operating policies, procedures, and protocols for all programs within the scope of responsibilities.

Uses effective service recovery skills to solve problems or service breakdowns when they occur.

COMBINING QUALIFYING EXPERIENCE AND EDUCATION (at the GS-9 and GS-11 level/s only)

Interviews and adjudicates requests for refugee status to determine benefit eligibility and determine final eligibility to the U.S. Refugee Admissions Program.

Researches immigration laws, policies, and relevant country conditions to analyze testimony, documentation, and credibility to determine eligibility for benefits.

Interpreting and applying immigration laws, policies and procedures as they relate to an individual's eligibility for immigration benefits.

Other Required Skills and Experience

1 year of community involvement experience preferred

Coordinates meetings for CHW's to communicate and share best practices. Coordinates meetings with Clinical liaison.

High School diploma or equivalent preferred

Some positions may require that incumbents be bi-lingual in Spanish and English

Researcher – Refugee Resettlement Experience, Education, Skills, Abilities requested:

Promote data management plan requirements, creation, and implementation.

Preferred knowledge of disciplines such as refugee housing, legal and medical services for effective data management guidance implementation.

Experience with data management and data quality assurance.

Strong multitasking, organizational, and time management skills with minimal supervision.

Attention to detail and excellent time management skills required.

Education: Associate degree in, education, communication, or health related field or equivalent work experience

Supports area clinical team and serves as a proactive resource for members related to disease prevention and benefit education.

Experience: 2 years - Experience in a customer service support role is required

Proficient interpersonal and organizational skills

Independent, Sound decision-making and problem-solving skills

Self-motivated and able to manage multiple tasks and set priorities.
