Job Description


Refugee Services Health Navigator (RSHN) will work closely with Bureau of Refugee Services (BRS) Wellness Services Coordinator (WSC) to identify healthcare needs and coordinate services for clients enrolled in the Iowa Refugee Wellness Program (IRWP).


In coordination with WSC, RSHN will conduct Family Self-Sufficiency Plan (FSSP) with clients to assess needs. RSHN will develop self-sufficiency plans and timelines in coordination with WSC, and partnership with each client. RSHN will provide individualized support through direct services, referrals, and advocacy.


RSHN will ensure through a strengths-based case management approach that each client is familiar with contacting their primary & specialty providers, including scheduling their follow-up appointments.


RSHN works closely with WSC and responds to client health-related crises for those enrolled in the


IRWP.


In coordination with WSC, RSHN may perform home visits and/or accompany clients to appointments as needed.


RSHN will provide coordination and assistance with accessing medical care and following medical professional treatment recommendations until it is determined the refugee is able to address ongoing care needs independently.


RSHN may support efforts to link clients with appropriate educational, health, and social services. Assist clients to attain their established goals, address challenges, and utilize available resources.


RSHN may use personal, or Department of Administrative Services Fleet Services issued vehicle and/ or public transportation (where available) to travel and transport clients and materials as needed throughout the service delivery area.


RSHN may be called upon to provide interpretation services as appropriate, and applicable.


RSHN will actively participate in relevant trainings, and team meetings.


RSHN will coordinate with WSC, BRS Community Engagement Coordinator (CEC) and State Refugee Health Coordinator (SRHC) to provide clients with orientation on various topics regarding health, such as women's health and specialized treatment for people with physical disabilities to assist in navigating clients through the system.


In coordination with WSC, RHSN will ensure clients with identified mental health risks or interest in specialty mental health care have links and referrals to culturally and linguistically appropriate mental health care partners.


RSHN works in collaboration with WSC and refugee resettlement agency partners in providing care coordination for refugees whose pre-arrival information and/or initial refugee health screenings indicate needs for referrals to specific medical treatment.


RSHN works in coordination and collaboration with WSC, IRWP Team and SRHC to increase access to culturally and linguistically appropriate mental health services.


RSHN may carry out timely tasks-in coordination with community stakeholders, WSC and CEC-to ensure each client has living essentials such as housing, food, medical care, and access to transportation, and is oriented to their new environment.


RSHN will collaborate effectively with coworkers and partner organizations to build and maintain relationships with area service providers for the benefit of clients.


RSHN ensures monitoring and tracking of services and data collection through BRS database RIMS.


RSHN maintains digital client case records and files, including report creation.

Minimum Qualification Requirements


Experience equal to three years of full-time public contact work involving counseling, interviewing, referral, job development, and/or job placement, or related public contact work obtaining, analyzing and/or evaluating data;


OR


substitution of thirty semester hours of coursework at an accredited college or university for one year of the required experience to a maximum substitution of two years;


OR


employees with current continuous experience in the state executive branch that includes the equivalent of two years of specific public contact work at the Iowa Bureau of Refugee Services involving counseling, interviewing, job referral and/or placement shall be considered as qualified.

Additional Qualification Requirements
