Have strong project and people management skills

Demonstrated commitment to supporting communities who have experienced systemic oppression and bias.

Develop Epi Data Briefs, Vital Signs, conference abstracts, presentations, posters, and empirical manuscripts to further public health knowledge

Experience in survey development, design, implementation, analysis, and dissemination.

Knowledge of evaluation techniques, such as hypothesis development, applying theoretical frameworks, developing research questions and aims, and study design methodologies.

Experience incorporating racial equity and social justice principles in data and evaluation activities;

Experience with project management methodologies, such as Agile and Waterfall

Continuously improve project management processes and methodology

Identify and manage project risks and issues

Experience with data and analytics projects

Experience with Population Health Management programs as well as with EMRs or other clinical systems is ideal but not required.

Work directly with Providers and care team members both remotely and in-person across the MUSC Health Network

Assist in development and maintenance of Population Health Platform Knowledgebase

Provide training and support remotely and in person at corporate locations, business units, hospitals and/or practices throughout the MUSC Health network

Participate in and successfully completes Mandatory Education.

Experience with Epic Analytics and Epic Healthy Planet is ideal but not required.

Completion of supervised fieldwork and experience in case management, health, or behavioral health preferred

Conduct meaningful outreach, provide education, and drive member enrollment

Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, violence, and social issues is required

Working knowledge of care management, quality management and payor requirements.

Minimum of 3 years care management experience. Prefer experience with quality management and performance improvement.

Provide a clinical focus to quality management by assessing, planning, implementing and evaluating quality performance.

Duties and Responsibilities of this Level

Current licensure as a licensed clinical social worker by the NC Social Work Certification and Licensure Board

Requires ACM or CCM certification within 3 years of hire date.

Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

Prepares clients' discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.

Works alongside their team to plan, implement, and evaluate patient’s health needs.

Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.

Assist in patient care by assessing patient needs and developing a plan to help patients succeed after surgery.

Work with physicians to develop patient optimization and ensures ongoing care for patients is met.

Experience in Care Management, preferably in Behavioral Health.

Experience in crisis intervention for youth and adults.

A Master's Degree with Indiana licensure is REQUIRED:

Understanding of local domestic violence resources.

Ability to travel within the assigned region to meet with team and members as needed.

Bachelor's degree or 5 years of relevant experience

5+ years in Epic Population Health application build knowledge.

Responsible for the analysis, design, implementation, operation, deployment, and support of the Epic Beacon module

Leads various initiatives linked to EHR or process improvement.

Provides incident/issue support through data gathering.

Applied knowledge of program management and ability to lead and/or contribute to process improvement strategies

Strategically inform and provide guidance from a population health perspective to care management teams.

Understanding of managed care and publicly financed health care stewardship principles

Leadership experience in a managed care organization (preferrefed)

Minimum 5 years' related experience, including 3 years in a population health program

Minimum 2 years' experience in a supervisory position
