Be on-call to respond call/texts and are available for support clients in between visits.

Create and send reports/letters to referral sources and community agencies when needed.

Establish and maintain relationships with different referral sources and community organizations.

Get involved in promoting programs, marketing and engaging with the community.

Create and present both verbal and written reports to staff and relevant professional organizations as necessary.

Thoroughly reviews, assesses, and completes third party release recommendations related to family reunification options.

Visits care provider sites to conduct weekly staffing reviews.

Writes reports on progress and tracking/elevating issues related to program trends.

Assists care providers to recognize potential victims of trafficking.

Provides technical assistance to grantees related to policies and procedures

Bilingual proficiency in English and Spanish (will be tested)

Preferred: Electronic Practice Management system or Electronic Health Records system experience (EPM/EHR)

Practices a patient focused approach to healthcare as demonstrated by interactions showing respect, knowledge, responsibility, compassion and access to care

May train less experienced Heath Center team members in job functions/duties/protocols

Completes employee training for handling of situations involving conduct that endangers the welfare of minors, including mandatory reporting requirements

Fluency in Spanish/English required (if a job requirement it is documented in your employee records)

Strong organizational skills and ability to multi-task

Bilingual (English/Spanish) communication skills are required.

Able to effectively manage significant documentation.

Provides on-going consolation and assistance to resettlement partner staff.

Assist in securing suitable/affordable housing and coordinate the delivery of furniture/supplies to newly arriving refugee families.

Ensures language and cultural competency, mental health and social services in support of program.

Performs other job-related duties as assigned.
