Provides case management and advocacy in the areas of public benefits programs, education and other systems in the community

Completes forms, consistent with department policies and procedures, as well as State requirements

First Aid and CPR certifications

Develops treatment plans, collaboratively with clients, that are measurable and lead to positive behavior change, with periodic review and revision

Maintains case records and timely documentation on each client

Skill in developing and presenting training and educational programs.

Skill in maintaining records and files of activities for easy access and reference.

Skill in effectively presenting information and responding to inquiries or complaints from employees, patients and/or their representatives, and the general public.

Skill in the use of Microsoft Office Products including outlook, PowerPoint, EXCEL and Word.

Interprets and translates patient and family educational information.

Assists in research and development of topics related to language educational services and language acquisition.

2+ years previous experience working in care management and/or with CKD/ESRD patients is preferred.

Perform frequent daily in-home care management visits to execute care management plans

Review and document plan member updates and progress in care management platform

Ability to take call remotely on some nights and weekends

Must have Home Health and/or Hospice experience.

Thoroughly reviews, assesses, and completes third party release recommendations related to family reunification options.

Visits care provider sites to conduct weekly staffing reviews.

Writes reports on progress and tracking/elevating issues related to program trends.

Assists care providers to recognize potential victims of trafficking.

Provides technical assistance to grantees related to policies and procedures

Bilingual proficiency in English and Spanish (will be tested)

Show Proficiency in interpreting skills through one of the following: Certification, Degree in Interpreting/Translation, or Certificate in Interpreting/Translation

Principles and practices of administration and management

Previous experience interpreting for children and/or adults in paid or voluntary positions

Associates degree (or equivalent) or higher preferred, however not required

Regularly participates in professional development courses and workshops

Must be able to pass FBI and State background check
