Complete initial assessment of potential residents.

If clinical, may be required to perform patient visits and / or participate in on-call rotation. Required Experience/Skills:

Must have at least 1 year of home health experience.

Prior packet review / QI experience preferred.

Initiates infection control forms as needed, sends the HRD the completed "Employee Infection Report" to upload in the worker console.

Maintains the client hospitalization log, including entering coordination notes, and sending electronic log to all office, field, and sales staff.

Completes requested schedules for all add-ons and applicable orders:

Adheres to all policies of company, including job descriptions, certifications requirements, mission statement, HIPAA, confidentiality standards and patient rights

Provide leadership to and directly manage clinical therapy staff, facilitating a team atmosphere among staff

Provides in-service education to the members of the patient’s care team and education to the community

Meet the requirements of Health IT's Project Management Office.

Project management and software implementation experience

5+ years in a management role at a healthcare organization

Understanding of software development methodologies and change management

Strong understanding of health information security and risk management

Continually optimization solution in scope and ensure they meet user requirements.

3-5 years of progressive leadership experience in home health or a related healthcare setting.

Brightree Database experience is a significant plus

Veteran’s Services Contracts experience a plus

Strong knowledge of home health regulations, reimbursement systems, and industry trends.

Excellent analytical, problem-solving, and decision-making skills.

Exceptional communication and interpersonal skills, with the ability to build effective relationships with internal and external stakeholders.

Bachelor’s Degree in Nursing preferred or a minimum of 5 years of management experience.

Demonstrated ability to manage and supervise staff and experience in program planning and implementation.

Supervises the professional medical staff at Orange County Corrections Health Services including the Physicians, Dentist, Psychiatrist, Nurse Practitioners and Physician Assistants.

Assists the Medical Director in administrative functions including personnel issues, provider scheduling and peer review.

Participates in policy development and clinical protocol/treatment guidelines.

Collects, reviews and analyzes data for outcome studies. Compiles various statistical reports.
