Job Description

Salary: $60,000 (Annually)

Job Summary:

Hands Of Healing’s Lead Case Managers will oversee the implementation of services in accordance with ORR safe and Timely Release Policy. They are also responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file as per HOH’s policies and protocols. The Case Manager is required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours, weekends, and be on-call.

Essential Functions:

  • Collaborate with the Reunification Specialist to provide guidance and maintain current on ORR policy and release requirements to ensure timelines are met
  • Communicate all placement referrals with Lead Clinician and remain on call.
  • Attend all organizational required trainings.
  • Plan and coordinate internal departmental meetings, case staffing, repatriations, attorney-UC visits, LOPC appointments and referrals to the Young Center for Immigrant Children’s Rights.
  • Attend case staffing with funding source representative.
  • Ensure ORR safe and timely release policy revisions are appropriately implemented within required timeframes.
  • The LCM will provide oversight of the Case Management Department including all work submitted by the Co-Lead Case Manager, Case Manager, Reunification Specialist, Data Entry Clerks, Administrative Assistant, and the Reunification Records Clerk.
  • Submit daily reports to ORR including the daily capacity count (as maintained by YCWDECM), daily medical/special needs report, reunification and discharge reports and safe and timely release case status.
  • Serve as primary liaison between ORR, DHS, Border Patrol, legal services and Immigration Court.
  • Monitor compliance and implementation of program services.
  • Provide supervision and direction to Case Managers, conducts reviews of files to verify safe and timely release services are documented in the case file.
  • Ensure that the continuation of services, case management and reunification services will be provided 7 days a week.
  • Maintain ongoing communications with the PSA Compliance Manager to ensure all relevant in care and after care services are provided to UC, in accordance with ORR PSA Policy.
  • Manage the safe and timely release process, conduct monthly random file audits, maintain knowledge of all serious incidents (SIRs), review safety plans, assist in the coordination of the HOH Evacuation Process.
  • Attend training that will enhance professional growth in the area of Lead Case Manager and documentation, or other topics as deemed appropriate by the Program Director or Designee.
  • Attend all departmental and program meetings to ensure that up-to-date information is received and/or information of policy changes or practice are adhered to.
  • Travel as needed for trainings, conferences or to transport youth to destinations located within the U.S.
  • Maintain a safe, clean and hazard-free work area.
  • Ensure the proper supervision of youth at all times.
  • Must assist in the evacuation of youth as needed due to inclement weather conditions, natural disasters, or other unforeseen occurrences.
  • Able to react to change productively and handle other essential tasks as assigned.

Qualifications:

  • At minimum, the Lead Case Manager must hold a master’s degree Social Work, Psychology, Human Services, Counseling, or other social service field; or a bachelor’s degree and at least 3 years progressive employment experience that demonstrates supervisory and case management experience.
  • Must be computer literate with working knowledge of Microsoft Windows (Word Perfect, Microsoft Word, Excel)
  • Cleared Tuberculosis test results.
  • Must possess a valid state driver’s license and be eligible to drive to facilitate program services as required by Cooperative agreement.
  • Must be at least 21 years of age at the time of hire.
  • Cleared background check from appropriate entity.
  • Must be Bilingual (English/Spanish)
  • Cleared pre-employment and random drug test results.
  • Required to work a flexible schedule to facilitate program services.

Physical Demands:

Must be able to obtain and perform Emergency Behavior Intervention Training (EBI)/Crisis Prevention Institute (CPI) certification and First Aid Certification (CPR). Must be able to supervise clients indoors and outdoors as necessary. Bending, stooping and lifting up to 20 lbs. required to complete daily tasks. Job requires extended periods of sitting and use of various office equipment.

Work Environment:

Work is at a youth care facility at which minors receive multiple services. Two shifts per twenty-four-hour period. Work shifts are subject to change. Noise levels vary from mild to moderate. Predominantly indoors, occasional outdoor monitoring or activities.
