The Choices Case Manager must be knowledgeable about the various services available in the community and be able to connect clients to the appropriate resources. Detailed Job Description The Choices Case Manager is responsible for providing case management services to clients, including assessment, planning, coordination, monitoring, and evaluation of services. The Choices Case Manager must be knowledgeable about the various services available in the community and be able to connect clients to the appropriate resources.
