Requirements to include, but not limited to:

Handle both work items, outbound and inbound telephone calls, faxes, and emails to/from Financial Professionals

Bachelor's/Undergraduate degree or combination of education and 5+ years work experience in administrative case management support or related field

Child welfare and/or case management experience is strongly encouraged.

Obtain necessary documents from the UC Portal and prepare for prepare for insertion to Case Management reports

Computer and typing skills sufficient to perform essential job functions. Knowledge with Microsoft Office Suite is preferred.

Completes identified sections of the Home Study reports in collaboration with assigned case managers

Collaboratively work with direct supervisor and Case Managers to schedule and prioritize work duties

Two (2) years of full-time experience or part-time equivalent in a mental health setting.

Demonstrated knowledge of DSM-V and psychosocial rehabilitation and recovery principles.

Experience in working with health care and mental health regulatory agencies.

Prefer a bachelor's degree in psychology, social work, or a field related to mental health.

This is a remote position

Providing focused interventions for targeted patient populations, including, but not limited to: asthma; depression/anxiety; reproductive health/ high risk pregnancy care.

The ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.

Case Coordinator– will be responsible for:

Consultation and training (the work provides rehabilitation assistance in adjusting to independent community living and training in work skills and responsibilities)

Experience completing clinical and vocational behavioral assessments.

Experience with vocational rehabilitation services and community treatment for persons with long-term mental illness.

Ability to manage time and workload effectively.

Reports and evaluates in a professional manner within the time guidelines established by OCFS, to include:

Psychosocial assessments within 30 days of admission;

Service Plan Reviews on a quarterly basis;

FASPs on a 6 month basis or more as needed;

Provides insight and direction to management on consultant quality, availability and compliance with all company policies and procedures and client specifications.

Ability to follow instructions and respond to upper managements’ directions accurately.

Participate in various educational and or training activities as required.

Must have strong knowledge of medical terminology, anatomy and physiology, medications and laboratory values.

Must be knowledgeable of multiple software programs, including but not limited to Microsoft Word, Outlook, Excel, and the Internet.

Must possess excellent skills in English usage, grammar, punctuation and style.
