Mendota Mental Health Institute (MMHI) in Madison, WI is currently recruiting for three (3) Rehabilitation Case Coordinators.

Work hours consist of daytime hours, occasional evenings and weekends.

MMHI is conveniently located on the north side of Madison and offers a beautiful campus environment, convenient free parking and an onsite workout facility. MMHI is an inpatient psychiatric facility, with one outpatient program, specializing in serving patients with complex psychiatric conditions, often combined with certain problem behaviors. Position Summary


The Rehabilitation Case Coordinator is responsible for developing, coordinating and implementing comprehensive clinical plans for clients in the Program of Assertive Community Treatment (PACT). Responsibilities include:

  • Provision of direct clinical services
  • Provision of academic and vocational rehabilitation services
  • Consultation and training (the work provides rehabilitation assistance in adjusting to independent community living and training in work skills and responsibilities)
  • Development of a full range of vocational and independent living options in community settings
  • Consultation and technical assistance to other PACT staff on employment/rehabilitation issues and participation on various treatment teams in a vocational capacity

Salary Information


The starting salary is between $27.00 and $29.00 per hour (approximately between $56,160 and $60,320 per year), depending on qualifications, plus excellent benefits. A 12-month probationary period is required.

Pay for current or former state employees will be set based on the rules that apply to compensation for the applicable transaction. This position is in pay schedule and range 12/64.

Job Details

Qualifications


Minimally qualified applicants will have:

  • Met the qualifications for clinical coordinators under WI Administrative Code DHS 63.06(2)(c) OR meet at least one of the qualifications listed in (4)(a) 1-8 for Community Support Program (CSP) staff at the time of appointment.

Well-qualified applicants will also have one or more of the following:

  • Experience with vocational rehabilitation services and community treatment for persons with long-term mental illness.
  • Experience completing clinical and vocational behavioral assessments.
