Monday through Saturday from 8a to 8p (LPN or RN0

Seven nights a week - flexible 8 hour shifts (RN only)

Accurately follow an individualized plan of care

Monitor all aspects of a patient’s care

Consult and coordinate with health care team members to plan, implement, and evaluate patient care plans

Pay based on experience and specific case

Monday through Friday from 8:40a to 2:45p

Prepare and assist with examinations and treatments for patients

The LPN is responsible for utilizing the principles of Relationship Based Care while performing duties.

The LPN is responsible for initial data collection on all residents.

The LPN participates in implementing person driven care plans as developed by RN.

Current State licensure as a Practical Nurse, in good standing.

Must be able to read, write and communicate in English.

Monday through Friday 7a to 3p (or 8a to 4p)
