SSM Health has an exciting opportunity to join our Virtual Behavioral Health integration team. This is a transformational approach to identifying patients early and providing them the access and support they need to address their behavioral health issues BEFORE they become unmanageable. This position will work with patients and primary care physicians COMPLETELY VIRTUALLY. Job Summary:
Engages with patients and encourages them to take an active role in their health by providing them with the tools necessary to make healthy life choices and adopt lifelong health behaviors. Assist patient with coordination of care across the care continuum.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Provides customized, evidence-based patient education variety of areas including but not limited to weight management/exercise, tobacco cessation, stress reduction, and chronic disease self-management.
- Encourages the patient to participate in shared decision making with his/her doctor when faced with multiple treatment options. Helps the patient understand the options and evaluate personal preferences and values related to likely treatment outcomes, with the goal of preparing the patient to discuss the risks and benefits of each option with the physician.
- Advocates and facilitates referrals to gain access to services and resources for patients, including assistance programs, community-based services, and mental health support.
- Performs other duties as assigned.
- Implements a range of skills including but not limited to coaching, motivational interviewing, behavior modification, goal setting, barrier assessment and problem solving.
- Collaborates with patients to actively engage a population of medically vulnerable or chronically ill patients in their care, assisting them through the process of actively working towards better health by providing support, encouragement, and education.
- Works in a constant state of alertness and safe manner.
- Assists with the coordination of care across the care continuum (including home care, outpatient care, ER care, and hospital care).
- Communicates care plans and pertinent information with the primary care team as well as with other specialists or parties participating in the care of the patient.

- Bachelor's degree or equivalent years of experience and education

PHYSICAL REQUIREMENTS
- Occasional use of smell to detect/recognize odors.
- Occasional lifting/carrying and pushing/pulling objects weighing over 50 lbs.
- Rare crawling, running and lifting/moving patients.
- Constant use of vision for distances near (20 inches or less) and far (20 feet or more).
- Frequent standing, twisting and walking.
- Occasional keyboard use/data entry.
- Frequent lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Constant lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Occasional driving.
- Occasional bending, stooping, climbing, kneeling, sitting, squatting, reaching, gripping and repetitive foot/leg and hand/arm movements.
- Frequent use of speech to share information through oral communication.
- Frequent use of vision to judge distances and spatial relationships and to identify and distinguish colors.
- Constant use of hearing to receive oral communication and hear alarms, malfunctioning machinery, etc.
