Job Summary
RN Health Coach - work remotely from home - 4 day schedule - 9pm - 7:30am EST, 4 days, 10-hour shifts - $34.00/hour plus additional shift differentials - Spanish Speaking preferred NOTE: You must live in a compact state. PA and CA residents are not eligible for this position but you must hold a current license in one of these states The Health Coach provides unbiased evidence-based health information to assist individuals telephonically with any health related concern. The primary focus is to educate and guide a caller through their health care choices in order to be a more active and informed participant in the healthcare process. The includes general symptom management and answers to urgent questions, assistance and support in managing chronic or serious medical conditions, help preparing for important discussions with an individual's doctor, and general health related information and management.
Job Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES
The associate is responsible for the functions below, in addition to other duties as assigned:

Must reside in compact state

  • Attend and participate in team and/or department meetings. Participate in special projects as directed. (5%)
  • Educate, assess and guide a member through their health care choices by accessing evidenced based and approved tools and resources. Assist members in developing action plans to manage specific symptoms or conditions. (60%)
  • Participate in required trainings as assigned and support security and compliance related activities and task throughout execution of all job duties. (5%)
  • Maintain required credentials, per state requirements, for employment and additional certification required per business need. Complete continuing education to adhere to credential requirements and enhance clinical knowledge base to support personal and professional skills development. (5%)
  • Document interactions with members in the company’s coaching technology platform to capture member interactions and outcomes for member follow-up support, tracking and reporting. (20%)
  • Assist in the achievement of department and client goals by adhering to the coaching productivity and quality standards. Engage in the quality monitoring and review process to enhance clinical skills and support continuous quality improvement. (5%)

qualifications
To perform this job successfully, the associate must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required:
Education and/or Experience
  • Bachelor’s Degree in Arts/Sciences (BA/BS) in Nursing or health care related program preferred.
  • Associate’s Degree (AA) in health care related program or graduate from an accredited professional school or
college/university required.
  • 2 years of experience in working as a Registered Nurse in a clinical setting or equivalent health care related experience
required.
  • 2 years of experience in direct (hands-on) patient care in multiple nursing areas or disciplines such as med/surg, home
health, hospice, ER, critical care, etc. preferred.
  • 2 years of experience in providing telephonic triage to patients in a setting such as ER, hospice or afterhours preferred.
  • 2 years of experience in supporting patients with chronic conditions preferred.

Certificates, Licenses, and/or Registrations
  • Active Registered Nursing (RN) license in good standing; with ability to obtain and maintain multi-state (CA or PA RN license required) nursing licenses (where applicable), as dictated by business needs.
  • Required to reside and work in a state that is part of the Nurse Licensure Compact and hold multistate privileges. PA and CA residents are not eligible for this position but you must hold a license in one of these states.
  • CA or PA RN license required but cannot reside in these states

Language Skills
  • Ability to read and interpret documents, such as safety rules, general business periodicals, operating and maintenance
instructions/procedure manuals. Can speak effectively to customers. Able to type, prepare, copy, file, and keep records.
Pertinent knowledge, abilities, and/or training
  • Strong computer and keyboarding skills. Experience with Windows based software, Microsoft Outlook, Office and Excel
  • Sound critical thinking and clinical judgment
  • Multilingual skills desirable, based on business needs
  • Strong attention to detail
  • Strong mastery of English language; effective verbal and written communication
  • Strong communication skills with a demonstrated ability to educate individuals and drive behavioral change
  • Ability to adapt quickly to changing priorities and processes
