Social Worker - Patient And Family Services

To assist the patient, his/her family , or members of the medical health care team in meeting post-hospital health needs through appropriate teaching, psychosocial evaluation, and support and referral to community agencies that can provide services and treatment measures to restore the patient to his/her optimal physical and mental health.

Minimum Education: Master of Social Work or LCSW Preferred. Minimum Experience/Knowledge: Working knowledge of local, regional and federal health, social, and welfare resources Required License/Certification: Current BLS certification from the American Heart Association.
