Job Description
WANDER STAFFING is looking to add a Social Worker to our Team! This position is located at:


459 Patterson Road, Honolulu, HI 96819-1522 at the Pacific Islands Healthcare System.

Schedule:


  • This is a 52-week long Contract Position
  • Not to exceed 80 hrs over a two (2) week period.
  • Shift work Monday-Friday


Highlights:

Professional Requirements:


  • Master’s degree in field with a completed practicum
  • 2 years working with Veterans as a mental health counselor


Social Worker Duties And Responsibilities


  • Evaluate patients (existing, new and prospective) based on their needs, limitations, and desires
  • Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager
  • Build rapport with patients and their families
  • Develop care plans for patients
  • Coordinate care with PT and OT departments
