As a key member of our facility, you will play a crucial role in assisting, planning, organizing, and implementing the overall operation of the Social Services Department.
Responsibilities:
Conduct interviews with residents and families to obtain valuable social history, ensuring personalized care and support.
Engage residents and families in the planning of social service programs, empowering them to actively participate in their journey to recovery and well-being.
Provide innovative solutions for social and practical environmental challenges, including securing financial assistance, collaborating with community agencies, and making referrals when specialized assistance is required.
Collaborate with administration, medical, nursing staff, and other related departments to develop and execute effective social service programs and activities.
Participate in Facility Surveys conducted by authorized government agencies, reflecting our commitment to meeting and exceeding industry standards.
Effectively communicate the social, psychological, and emotional needs of residents and their families to the medical staff, attending physician, and the resident care team.
Requirements:
Minimum of 1-year experience working in a Skilled Nursing Facility, demonstrating a strong understanding of the unique challenges and dynamics of this environment.
