Position Summary


The Social Worker will play a vital role in reaching out to members to identify social determinants of health (SDoH) and resulting potential barriers to care. Through collaboration with Care Managers - RNs, the Social Worker will facilitate referrals to appropriate community-based resources and organizations, aiming to improve member healthcare outcomes.

Responsibilities
  • Collaborate with external agencies to ensure seamless continuity of care for members
  • Utilize established assessments to collect information and identify psychosocial needs, and social barriers to care, which will inform the care planning process conducted by the Care Manager
  • Other related duties as assigned
  • Educate and guide members on the psychosocial aspects of chronic conditions among the adult population, including the effects on behavioral health (e.g., grief, depression, anxiety, etc.)
  • Offer interventions that may include education, guidance, and support to address SDoH barriers (housing, finances, transportation, and mental health/behavioral health needs)
  • Engage members telephonically based on referrals from the Care Management team, promoting their active involvement in the care process
  • Encourage adherence to the provider's treatment plan and the Care Manager's plan of care
  • Actively participate in team meetings, multidisciplinary team meetings, in-service trainings, and regular 1:1 meetings with a direct manager
  • Be accountable for meeting individual metrics related to the Social Worker role, as defined by the department leads
  • Document all relevant information in the company’s proprietary care management systems
  • Conduct necessary behavioral health assessments
  • Facilitate connections to appropriate community health and human services, assisting in locating wrap-around services available within the member's community
Requirements
  • Fluency in English & Spanish
  • Possess at least 3 years of experience in a healthcare or human services agency setting
  • Prior experience working in the community, with knowledge of renal health, dialysis, and/or community resources, is advantageous
  • On Island Residency
  • Hold a Master's degree in related Human Services/MH, SW (Social Work); an active current license as LCSW (Licensed Clinical Social Worker) or LMHC (Licensed Mental Health Counselor) is preferred
  • Experience in a managed care setting is desirable
Skills
  • Must have experience in company managed systems (HRIS, proprietary software)
  • Capable of independently identifying problems, clarifying facts, and organizing information to recognize trends, information gaps, or the need for additional information
  • Strong interpersonal skills to effectively motivate patients and other stakeholder
  • Proficiency with MS Office and automated database management systems (Excel, Access, PowerPoint)
  • Excellent written and verbal communication skills
  • Must have experience in virtual meeting applications (zoom, ring central/etc)
  • Demonstrate cultural awareness and knowledge
  • Strong organizational abilities to manage multiple responsibilities efficiently
