:

Gather psychosocial information to participate in the development and implementation of a plan of care that provides extraordinary care for the patient while being sensitive to costs and resources. Work collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties. The SSW will execute these duties under direct clinical supervision.

Position Details:

This role work in the Intake department, where incumbents will work closely with a tightknit team who provide support for each other and make the job fun! This position will be taking calls from parents helping them navigate the Behavioral Health field and find services for their child. Incumbent will also be coordinating with in-network providers looking for behavioral health services for existing clients. It’s very rewarding when parents express appreciation for finding them the help their child needs.

Shifts for this position are Monday-Friday from 8:30 am 5 :00 pm or 9 :00 am– 5:30 pm.

Minimum Qualifications:

• Current state licensure as a Social Services Worker.
• Bachelors degree from an accredited institution (degree will be verified).

Preferred Qualifications:

• Experience working with in a healthcare setting.
