If you are located in TN, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:
- Support the design, planning and implementation of an organizational-wide DEI strategy that is embedded into all aspects of the Enterprise
- Collects data on social impact opportunities and potential strategic partners for possible grant opportunities in the state of Tennessee
- Works with the entire population health team to meet goals and solve problems related to social needs and health equity
- Assists in project management of the social needs and health equity initiatives
- Performs other related functions as assigned
- Identifies and reports on health inequalities and helps develop strategies and interventions to provide health equity to all members
- Comfortable leading or facilitating meetings with multiple stakeholders
- Participates in continued education in social responsibility, social determinants, and all other necessary education
- Works on special projects related to social determinants of health, rural health strategy and requests from TennCare
- Works closely and develops relationships with leadership, members, providers, and other areas to ensure coordination, efficiency, and enhanced collaboration
- Reports and participates on community initiatives that are relevant to our members
- Support organizational DEI change readiness and management efforts
- Supports relationships with external vendors which includes submitting invoices for payment for vendor services
- Works across the health plan to help implement and monitor rural health strategies
- Supports health plan community investment project ensuring equitable distribution of funds
- Involves some time out of the office and traveling
- Supports reporting, auditing, and process performance validation activities
- This position may require cross-training with other positions
- Executes activities designed to target gaps in care related to social risk factors and health equity while working to achieve individual and team targets
- Plans, coordinates and reports on health promotion and wellness education outreach activities for the UnitedHealthcare Community and State plan in Tennessee
- Support and collaborate internally with staff and leadership to drive awareness, commitment and accountability for the DEI action plans across the organization
- Develops relationships with community organizations and providers to close gaps and allow for warm hand offs of members requiring community resources
- Monitors Social Determinants of Health and strategically plans with community partners and providers how to remove barriers (food insecurity, housing insecurity, etc.)
- Prioritizes and organizes own work to meet deadlines
- Plans ways to impact members through community relationships which lead to better lives and healthier communities
- Applies principles of continuous quality improvement to all processes and projects

Required Qualifications:
- 4+ years of experience in a deadline driven environment
- 4+ years of experience managing and prioritizing several deliverables
- Must reside in the State of Tennessee
- Customer or provider engagement experience
- Must be willing to attend meetings, community activities/events across Tennessee; sometimes before or after standard work hours and on weekends
- Bachelors degree in healthcare, public health, social work, or similar field or equivalent experience in health care or social work
- Intermediate (or higher) level of experience with Microsoft Office (PowerPoint, Word, & Excel) including the ability to edit, create, save, and send spreadsheets and documents
- Ability to travel across Tennessee
Preferred Qualifications:
- Proficient in public speaking and networking in the community
- Project Management Experience
- 4+ years of proven experience influencing and collaborating with others
- Knowledge of Social Determinants of Health
- Demonstrated ability to create qualitative and quantitative reports and solid analytical skills
- Provider engagement experience
- Experience collaborating with community partners
- 1+ years of experience with Medicare and/or Medicaid
