General Summary

Our team is seeking a full-time, 100% remote Registered Nurse Team Lead to help support a few of our offices, including an Adult Primary Care Office in Westwood. This is an exciting opportunity to perform ambulatory phone triage and help our patient population while working from home. The desired schedule is Monday through Friday from 8 am to 5 pm.

Reporting to the Nurse Practitioner Team Lead, the Registered Nurse Team Lead will be working with other remote RNs to perform phone triage for complex urgent calls in a busy Internal Medicine Practice. The RN Team Lead will be responsible and accountable for planning and providing patient care for assigned patients in accordance with Nursing Standards of Care and organizational policies. The RN Team Lead will collaborate and work closely with physicians, associate providers, clinical and administrative staff to enhance the delivery of patient care services along the continuum of care. In the future, there is the opportunity for an in-office presence, focusing on patient education and chronic care management

• Acts as a patient advocate, investigating causes of complaints and taking corrective action as needed.
• Oversees the nursing triage program including holding regular meetings and conducting annual performance evaluations with input from NP Team Lead.
• Reviews team members’ clinical documentation for completeness, accuracy and consistency to ensure compliance and adherence with established criteria.
• Works independently on regular and/or assigned projects.
• Identifies clinical and operational issues and communicates to Manager as needed.
• Identifies clinical staff education and training needs and coordinates training as appropriate.
• Communicates patient care needs to the clinicians.
• Assesses and coordinates nursing triage staffing needs, including onboarding of new triage nurses.

Qualifications

• Graduate of an approved school of nursing
• Epic proficiency preferred
• Current Massachusetts RN licensure
• At least 3 years of adult primary care experience preferred
