The RN Pediatric Care Coordinator is responsible for assessing, tracking, implementing and coordinating routine pediatric care for the Auburn clinic of Chapa-De Indian Health. In addition, this position is responsible for conducting regular VFC immunization clinics and coordinating immunizations. This role is specific to addressing the needs of pediatric patients. This position proactively provides case management and clinical coordination for the clinics pediatric population.


RN Pediatric Care Coordinator


  • Assist with the management of pediatric patient care and act as a staff resource as needed during the day.
  • Assume the role of clinics Vaccines For Children (VFC) coordinator. Responsible for ensuring clinic is meeting current VFC guidelines, taking vaccine inventory, administrating vaccines, ordering vaccines, and overseeing vaccine storage.
  • Assist in developing a well child group visit program.
  • Contact patients after discharge from acute care facilities, visits to emergency departments, or urgent care centers and coordinate follow-up visits
  • Utilize reports of pediatric patients with needs past due, coordinate outreach to remind parents or clinicians proactively of services needed
  • Triage responsibilities include: patient triage, telephone advice, and patient education for pediatric patients. Assist with adult patient triage when triage volumes are high or staffing shortage.


Required Experience


MINIMUM QUALIFICATIONS:


  • Current and in good standing; CDL, vehicle insurance, and DMV printout.
  • Demonstrated ability to work independently, when needed and collaboratively with other members of the health care team.
  • Sensitive to the health and cultural needs of Native Americans and vulnerable populations.
  • Computer literacy in Microsoft Word & Excel required.
  • RN License, current and in good standing with the state of California, BSN Preferred.
  • Current CPR Certificate.
  • Previous pediatric experience required.
  • A minimum of two years current nursing experience.
  • Must have received or be willing to receive the COVID - 19 vaccine before date of hire to be considered for employment.
