Job Description
Manages individualized, goal directed nursing care through use of the nursing process and the principles of primary nursing in accordance with departmental and Hospital policies and procedures. The Registered Nurse will provide care that includes physical assessment, treatment, reassessment, discharge planning, and health education to the hospitalized patient. The RN is responsible for planning and coordination of care across the continuum. This planning and coordination includes meeting and exceeding compliance with core measures, contributes to reduction in LOS, active participation with all aspects of quality, safety and performance improvement activities, collaboration with all the members of the multidisciplinary team in providing outstanding customer service to patients, family members, physicians and the public. Meets and exceeds the Department of Health Service (DHS), The Joint Commission (TJC) and other regulatory requirements. The Registered Nurse will support the department initiative and provide quality patient care.


Assesses and diagnoses patient and family needs in order to provide quality care to assigned patients.


Develops. discusses and communicates a realistic problem list (plan of care) for each patient, in collaboration with each patient/family/significant other in order to address all identified needs.


Demonstrates the skills and judegment necessary to implement medical plan of care, nursing inverventions and procedures as necessary for the care of the patient.


Evaluates the identified problems. care provided and patient's responses are reassessed in order to meet patient and family needs to assure overall quality of care delivered.


Maintains up-to-date and accurate documentation of nursing care provided to assure the integration of information for use by the health care team, in order to assure quality care.


Maintains current knowledge/certification/licensure. Pursues professional growth and developement.


Functions as a role model for current and new staff.


Performs all resonsibilities/duties required by the subacute unit as defined in the scope of service to assure that the unique nature of the client is addressed. This includes, but is not limited to the age of the client served.


Attends all required safety training programs and can describe his/her responsibilities related to the general safety, department/service safety, specific job-related hazards.'


Demonstrates respect and regard for the dignity of all patients, family, visitors and fellow employees to ensure professional, responsible and courteous environment.


Promotes effective working relations and works effectively as part of a department/unit team inter and intra departmentally to facilitate the department's/unit's ability to meet its goals and objectives.


Assues All Other Duties And Responsibilities As Necessary.


Minimum education


Graduate of an accredited school of nursing


Minimum work experience


At least 6 months experience in general acute care


facility or long term care/skilled nursing facility


Required Licenses/Certifications


Current California RN license


Current BLS certificate


Requied Skills, Knowledge, and Abilities


Demonstrated proficiency in acute-care nursing, knowledge, and skills.


Maintains professional knowledge and skills related to ventilator dependent population.


Computer literacy and adaptability required.


Proficiency in both written and oral English


Bilingual preferred


This position is responsible for performing a variety of radiographic, fluoroscopic and specialized procedures at a technical level not requiring constant supervision, under the general direction of a radiologist and Radiology Administration. Collaborating with department and multidisciplinary team to ensure quality patient care and department service applying AHMC Core Values to the performance of every job duty.


Image Quality Consistently produces quality x-ray, MRI and CT images within hospital guidelines and department standards (i.e. patient safety, repeat rate, exam protocols, film processing, etc.


2 Documentation Accurately completes documentation according to hospital and department guidelines, including exam performed, billing, film labeling, computer entry, etc. Performs other clerical duties as necessary (place orders, film files, schedule patients, etc.)


3 Clinical, Information Obtains patient clinical information which may affect diagnostic testing including allergy history, pregnancy, etc. Documents and communicates this information to the appropriate clinical staff.


4 Competency/Training Maintains clinical competency assuming responsibility for upgrading skills through participation in in-service education and other continuing education activities. Participates in the training of departmental personnel and students.


5 Infection Control Assists MD and other staff in preparing for and performing procedures consistently, following sterile technique, infection control guidelines and department standards.


6 Work Flow Evaluates personal and department work volume and flow to insure efficient departmental patient care. Prioritizes work according to urgency and resources. Responds effectively in emergency situations. May act as supervisor in absence of department supervisor.


7 Quality Control/Quality Improvement Understands and implements department quality control/quality improvement plans performing QC/QI activities as assigned.


8 Resource Utilization In all duties, uses supplies and resources efficiently and cost effectively. May be responsible for specific inventory and stocking of supplies. Maintains cleanliness and orderliness of department.
