Current Basic Life Support for Health Care Provider Certification/CPR

Minimum one year work experience in hospice, home health, geriatrics, or medical/surgical nursing

Attends case conferences, team meetings and education in-services, as required

One year prior professional nursing experience preferred

Must meet all federal, state, local and contract requirements

Assesses and evaluates patient nursing needs regularly

Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required

Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change

Experience working in pediatrics not necessary

Novice Nurse and Skills Advancement programs allow nurses to advance their skills under the mentorship of experienced Clinical Supervisors

Provide skilled nursing care to pediatric and/or geriatric patients in the home setting

Provide health education to patients and caregivers

RN, home health or Private Duty Nursing experience preferred

Pediatric, Trach, and Vent experience preferred

Paid Training and Free Yearly Courses, which may be applied to the Illinois Board of Nursing’s CE Requirements

Attends case conferences, team meetings and education in-services, as required

Stripping, mopping, carpet cleaning, buffing, vacuuming and sweeping of floors

Cleans all bathrooms, rest rooms, sinks and utility areas.

Maintains a clean and safe environment.

Assists clients/ caregivers in learning appropriate self-care techniques.

Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals.

Prepares clinical progress notes and follows client care pans as developed by the registered nurse and according to the agency policy.

Assist the physician and registered nurse in performing specialized procedures for which competency has been established.

Prepares equipment and materials for treatments observing aseptic technique as required.

Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.

Assist with the management of pediatric patient care and act as a staff resource as needed during the day.

A minimum of two years current nursing experience.
