Rn Home Health - Remote | Wfh

We are currently seeking a skilled Home Health RN to join our team in supporting service areas for our Lincoln, Nebraska branch. As a member of our team, you will be offered a $5K sign-on bonus, new market rates, and the flexibility of a no on-call requirement.
Your day-to-day responsibilities will be varied and dynamic, with the opportunity to work flexible schedules that consist of 4-day workweeks with alternating 10-hour day shifts. You will perform comprehensive patient assessments and collaborate with a multi-disciplinary team, including PT, OT, ST, MSW, and paraprofessionals, to ensure your patients achieve their desired outcomes and goals.
As part of our team, you will also have the opportunity to implement and drive appropriate nursing clinical programs and initiatives, utilizing technology and telehealth to advance patient goals and outcomes. You will supervise a caseload that includes LPNs and Home Health Aides and facilitate care coordination with PRN staff when necessary.
Your communication skills will be essential, as you will need to document accurately and in a timely manner using a tablet, including OASIS assessments, SOC/Admissions, recertification's, physician orders, etc. Additionally, you will be expected to collaborate and coordinate care with primary and specialty healthcare providers, as well as participate in collaborative team and case conferences, leading discussions on multi-disciplinary cases when appropriate.
To be considered for this role, you must hold an active RN license, have a minimum of one year of clinical experience as a Registered Nurse (RN), and possess a valid driver's license.
