Remote Medical Assistant ($18.00 - $20.00 / Hour)

Responsibilities

:

• Collaborates with hospitalists and discharge planners to determine post-hospital care needs.
• Ensures post-hospital visits are scheduled with primary care providers.
• Documents all calls using the Outreach template.
• Visits patients in the hospital to assist with discharge care and transition of care.
• Documents admission and discharge information using the CC admission and discharge template.
• Records admission and discharge diagnoses, abnormal test results, medication changes, consult templates, and discharge plans.
• Conducts follow-up calls post-hospital to ensure office visits are scheduled, and performs additional outreach if patients fail to complete office visits.
• Establishes personal rapport with potential and actual patients and other individuals involved in patient care to understand their requirements.
• Reviews the daily hospital census for in-patient admits and ER visits, including the weekend census on Mondays.
• Reviews hospital records and sends notes to primary care clinicians regarding discharged patients from the weekend.

Skills

• Proficiency in electronic health records (EHR) and basic computer skills.
• Strong interpersonal skills to establish and maintain effective relationships with patients, medical staff, and the public.
• Capability to interpret, adapt, and apply guidelines and procedures.
• Knowledge of professional nursing theory and practice to provide and evaluate patient care.
• Familiarity with organizational policies, regulations, and procedures related to patient care administration.
• Ability to apply and modify principles, methods, and techniques of professional nursing to deliver ongoing patient care.
• Ability to remain calm and effective in emergency situations.
