Job Description
New Star is a non-profit serving over 650 children and adults with disabilities throughout the Chicagoland area and Northwest Indiana who has successfully provided services for over 70 years. Position: QIDP Program Manager
Salary: $36,000 to $41,000
This position is responsible for providing oversight and management to consumers receiving Medicaid Waiver services.
Essential Duties and Responsibilities include the following, although other duties may be assigned to meet the agency needs:
  • Provides ongoing training/support to develop staff members and monitors performance by developing improvement plans and evaluation reports as needed.
  • Hires, trains and supports staff members to provide individualized support to each consumer based on their wants, strengths, needs and goals.
  • Develops staff schedules and fills in as needed.
  • Completes monthly billing for Medicaid Waiver services.
  • Works closely with the team to complete and/or review assessments to support the development of the Person-Centered Individualized Support Plan (PCISP). These assessments include, but are not limited to the risk assessment, natural support assessment, rights assessment and Personal Outcome Measures.
  • Prepares monthly reports documenting services provided and progress towards outcomes developed by the team to support consumers to meet their goals and shares them with the team through the BDDS portal.
  • Maintains consumer files to ensure they meet DDRS and CQL standards.
  • Develops relationships with consumers and their team members to effectively provide services that meet the needs of each consumer and allows them to live their best life
  • Advocates for all consumers by protecting their rights, helping them voice their concerns and providing them with support and training on advocating for themselves and others.
Physical Requirements:
  • Must be able to safely bend, twist, push, pull and reach.
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Must be able to safely lift as needed.
Qualifications:
  • Excellent verbal and written communication skills
  • At least five years of experience working with individuals with disabilities
  • Bachelor’s degree in Human Services field a plus
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices
  • Organizational and Time Management skills
  • Ability to effectively work with consumers, outside providers, and family/guardians
  • Must maintain a valid driver’s license and meet agency driving requirements
  • At least three years of supervisory experience
  • Excellent computer proficiency (MS Office- Word, Excel and Outlook)
